摘要:
目的 评估利多卡因与芬太尼对急性肾绞痛患者的镇痛效果，为临床选择合理的镇痛方式提供参考依据。方法 选取2013年6月~2018年6月我院收治的肾绞痛患者120例，随机分为利多卡因组、芬太尼组，各60例，两组分别静脉注射对应的镇痛药物。比较两组注射后5、10、15、30 min视觉模拟疼痛评分（VAS）、疼痛程度、脉搏、血氧饱和度、血压、不良反应及止痛效能。结果 两组注射后5、10、15、30 minVAS评分、疼痛程度、脉搏、血氧饱和度及血压比较，差异无统计学意义（P＞0.05）；COX比例风险模型显示，与芬太尼组相比，利多卡因组治疗15 min和30 min止痛无效的绝对风险分别增加了26.70（95% CI：8.31~44.93）和4.41（95% CI：13.34~22.26）；两组胸闷、发热、心悸发生率比较，差异无统计学意义（P＞0.05），芬太尼组恶心、呕吐、腹胀发生率高于利多卡因组，差异有统计学意义（P＜0.05）。结论 急性肾绞痛患者静脉注射芬太尼与利多卡因均对患者生命体征无较大影响，与静脉注射利多卡因相比，芬太尼能够更迅速的控制疼痛，但是可能增加消化道不良反应。
Abstract:
Objective To evaluate the analgesic effect of lidocaine and fentanyl on patients with acute renal colic and to provide a reference for clinically selecting a reasonable analgesic method. Methods A total of 120 patients with renal colic treated in our hospital from June 2013 to June 2018 were randomly divided into the lidocaine group and the fentanyl group, with 60 cases in each group, and the two groups were injected with corresponding analgesics. Visual analogue pain scores (VAS), pain, pulse, blood oxygen saturation, blood pressure, adverse reactions, and pain were compared between the two groups at 5, 10, 15, and 30 min after injection.Results There was no significant difference in VAS score, pain, pulse, blood oxygen saturation,and blood pressure between the two groups at 5, 10, 15, and 30 min after injection (P>0.05).There was no significant difference in the incidence of chest tightness, fever, and palpitations between the two groups(P>0.05).The incidence of nausea, vomiting and bloating in the fentanyl group was higher than that in the lidocaine group,the differences were statistically significant (P<0.05).Conclusion Intravenous fentanyl and lidocaine in patients with acute renal colic have no significant effect on patients’ vital signs and can effectively control pain, but intravenous fentanyl can increase gastrointestinal adverse reactions.
