摘要:
目的 比较宫颈癌术后患者辅助放疗过程中容积旋转调强(RapidArc)和适形调强放疗(IMRT)的剂量学、近期毒副反应之间的差异。方法 收集2019年1月~2020年6月于安徽医科大学第一附属医院放疗科进行治疗的经病理确诊的宫颈鳞癌患者51例作为研究对象,根据辅助放疗方式分为RapidArc组(31例)和IMRT组(20例),比较两组靶区剂量学参数均匀性指数(HI)、适形性指数(CI)、机器跳数(MU)、危及器官受量及毒副反应。结果 RapidArc组MU小于IMRT组[(614.00±91.75)次vs(1576.35±418.70)次],差异具有统计学意义(P<0.05);两组CI、HI比较,差异无统计学意义(P>0.05);RapidArc组直肠V40大于IMRT组[(44.45±4.18)% vs(40.54±5.22)%],骨盆V20[(77.00±5.55)% vs(82.78±4.36)%]和V30[(47.21±9.15)% vs(52.43±6.86)%]小于IMRT组,差异具有统计学意义(P<0.05);RapidArc组皮肤副反应小于IMRT组,差异有统计学意义(P<0.05)。结论 宫颈癌术后容积旋转调强放疗过程中机器跳数小于适形调强放疗,该法治疗时间短,对骨盆的保护作用强,患者副反应轻,安全有效,值得临床应用。
Abstract:
Objective To compare the dosimetry, recent toxicity, and side effects of volume rotational intensity modulated (RapidArc) and conformal intensity modulated radiotherapy (IMRT) during adjuvant radiotherapy for patients with cervical cancer.Methods A total of 51 patients with cervical squamous cell carcinoma diagnosed pathologically who were treated in the Department of Radiotherapy of the First Affiliated Hospital of Anhui Medical University from January 2019 to June 2020 were collected as the research objects.According to the method of adjuvant radiotherapy, they were divided into RapidArc group (31 cases) and IMRT group (20 cases).The two groups were compared with the uniformity index (HI), conformity index (CI), monitor units (MU), exposure to organs at risk and toxic and side effects of the two groups.Results MU [(614.00±91.75) times vs (1576.35±418.70) times] in the RapidArc group was smaller than that in the IMRT group,the difference was statistically significant (P<0.05);There was no significant difference in CI and HI between the two groups (P>0.05);V40 of rectum[(44.45±4.18)% vs (40.54±5.22)%] in RapidArc group was greater than IMRT group, V20 of pelvic[(77.00±5.55)% vs (82.78±4.36)%] and V30 [(47.21±9.15)% vs (52.43±6.86)%] was smaller than the IMRT group, the difference was statistically significant (P<0.05);The incidence of acute radiodermatitis in Rapidarc group showed lower than in TMRT group, the difference was statistically significant (P<0.05).Conclusion The monitor units of RapidArc for adjurant radiotherapy of cervical cancer is less than that conformal IMRT. This method has a short treatment time, strong protective effect on the pelvis, mild side effects, safe and effective, and is worthy of clinical application.
