摘要:
目的 观察舒适护理与全面护理改善ICU慢性阻塞性肺疾病并肺心病患者肺部感染的效果。方法 选取2019年4月~2020年4月在我院诊治的68例ICU慢性阻塞性肺疾病并肺心病患者为研究对象，采用随机数字表法分为对照组和观察组，各34例。对照组采用常规护理，观察组采用舒适护理联合全面护理干预，比较两组患者急性生理与慢性健康状况（APACHEⅡ）评分、舒适度评分、血常规指标[白细胞分类计数（WBC）、中心粒细胞比例（NEU）]、降钙素原（PCT）、呼吸频率、氧合指数、护理满意度、肺部感染率以及病死率。结果 干预后，两组APACHEⅡ评分均低于干预前，舒适度评分高于干预前，且观察组APACHEⅡ评分低于对照组，舒适度评分高于对照组（P＜0.05）；干预后，两组WBC、NEU、PCT水平均低于干预前，且观察组低于对照组（P＜0.05）；干预后，两组呼吸频率低于干预前，氧合指数高于干预前，且观察组呼吸频率低于对照组，氧合指数高于对照组（P＜0.05）；观察组护理满意度为94.11%，高于对照组的85.29%（P＜0.05）；观察组肺部感染率为5.88%、病死率为2.94%，均低于对照组的11.76%、8.82%（P＜0.05）。结论 舒适护理与全面护理干预有助于改善ICU慢性阻塞性肺疾病并肺心病患者呼吸频率，降低APACHEⅡ评分，提高患者舒适度，降低WBC、NEU、PCT水平，预防肺部感染，提高护理满意度，降低病死率，护理效果确切，值得应用。
Abstract:
Objective To observe the effect of comfort nursing and comprehensive nursing in improving lung infection in patients with chronic obstructive pulmonary disease and pulmonary heart disease in ICU. Methods A total of 68 patients with chronic obstructive pulmonary disease and pulmonary heart disease in the ICU who were diagnosed and treated in our hospital from April 2019 to April 2020 were selected as the research objects. They were divided into the control group and the observation group by random number table, with 34 cases in each group.The control group used conventional nursing care, and the observation group used comfort nursing combined with comprehensive nursing intervention. The two groups were compared with acute physiological and chronic health (APACHEⅡ) scores, comfort scores, blood routine indicators [white blood cell differential count (WBC), centrogranulocyte ratio (NEU)], procalcitonin (PCT), respiratory rate, oxygenation index, nursing satisfaction, lung infection rate, and case fatality rate.Results After the intervention, the APACHE Ⅱ scores of the two groups were lower than before the intervention, and the comfort score was higher than that before the intervention. The APACHE Ⅱ score of the observation group was lower than that of the control group, and the comfort score was higher than that of the control group (P＜0.05) ；After the intervention, the WBC, NEU, and PCT levels of the two groups were lower than before the intervention, and the observation group was lower than the control group(P＜0.05);After the intervention, the breathing frequency of the two groups was lower than that before the intervention, the oxygenation index was higher than that before the intervention, and the breathing frequency of the observation group was lower than that of the control group, and the oxygenation index was higher than that of the control group(P＜0.05);The nursing satisfaction rate of the observation group was 94.11%, which was higher than 85.29% of the control group(P＜0.05);The pulmonary infection rate in the observation group was 5.88%, and the fatality rate was 2.94%, both lower than the 11.76% and 8.82% in the control group(P＜0.05). Conclusion Comfort nursing and comprehensive nursing intervention can help to improve the respiratory rate of ICU patients with chronic obstructive pulmonary disease and cor pulmonale, reduce APACHE Ⅱ score, improve patient comfort, reduce WBC, NEU, and PCT levels, prevent lung infections, and improve nursing satisfaction, reducing the mortality rate,the nursing effect is accurate, and it is worthy of application.
