目的 探讨脑白质高信号(WML)与颈动脉易损斑块的关系。方法 收集2012年1月-2019年12月就诊于克拉玛依市中心医院的颈动脉斑块患者238例,均完善头颅MRI、生化及血清Hcy水平,通过Fezakas量表评价WML评分,将其分为轻度及以下脑白质高信号组(0~1分)和中重度脑白质高信组(2~6分),比较两组年龄、性别、高血压、糖尿病、高脂血症、吸烟、饮酒、Hcy及易损斑块,分析WML评分与各项指标的相关性,Logistic回归分析WML的危险因素。结果 两组性别、糖尿病、高脂血症、吸烟、饮酒比例及Hcy水平比较,差异无统计学意义(P>0.05);两组年龄、高血压及易损斑块比例比较,差异有统计学意义(P<0.05);颈动脉斑块患者WML评分与年龄、高血压病及颈动脉易损斑块呈正相关(r=0.416、0.147、0.319,P<0.05);Logistic回归分析显示,年龄、高血压病、颈动脉斑块是WML的独立危险因素。结论 颈动脉易损斑块、年龄、高血压是脑白质高信号的独立危险因素,为降低脑白质高信号的发生率,除了关注患者年龄、高血压等常见危险因素,还需要常规筛查颈动脉超声,及早干预患者颈动脉易损斑块,降低脑白质高信号及症状性脑梗死的发生率。
Abstract:
Objective To investigate the relationship between cerebral white matter high-signal and vulnerable plaque of carotid artery.Methods A total of 238 patients with carotid plaques diagnosed in Karamay Central Hospital from January 2012 to December 2019 were collected, and all of them were performed MRI, biochemical and serum Hcy levels, the WML score was evaluated by the Fezakas scale, and it was divided into the mild and below white matter high-signal group (0-1 score) and the moderate and severe white matter high-signal group (2-6 scores). The age, gender, hypertension, diabetes, hyperlipidemia, smoking, drinking, Hcy and vulnerable plaques of the two groups were compared. The correlation between WML score and various indicators was analyzed. The risk factors of WML were analyzed by Logistic regression.Results There was no significant difference in gender, diabetes, hyperlipidemia, smoking, drinking proportion and Hcy level between the two groups (P>0.05). There was significant difference in age, hypertension and vulnerable plaque ratio between the two groups (P<0.05). The WML score of patients with carotid plaque was positively correlated with age, hypertension and carotid vulnerable plaque (r=0.416, 0.147, 0.319,P<0.05). Logistic regression analysis showed that age, hypertension and carotid plaque were independent risk factors for WML.Conclusion Carotid artery vulnerable plaque, age, hypertension are independent risk factors of cerebral white matter high-signal, in order to reduce the incidence of white matter high signal, in addition to paying attention to common risk factors such as age and hypertension, routine screening of carotid ultrasound is needed to intervene in patients with carotid vulnerable plaque as early as possible to reduce the incidence of high white matter signal and symptomatic cerebral infarction.
