摘要:
目的 飞秒激光白内障手术和传统超声乳化手术在眼相关高阶像差的对比研究。方法 选取2020年1月-2021年3月我院住院行白内障手术的患者180例(180眼),采用随机数字表法分为两组,每组90例(90眼),分别行飞秒激光辅助白内障手术组(飞白组)和传统超声乳化白内障手术组(传统组),比较两组手术前后的角膜、眼内(慧差和三叶草像差)、全眼高阶像差。结果 两组术后全眼和眼内高阶像差均低于术前,差异有统计学意义(P<0.05);飞白组术后角膜高阶像差与术前比较,差异无统计学意义(P>0.05);传统组术后角膜高阶像差高于术前,差异有统计学意义(P<0.05);飞白组术后全眼、角膜高阶像差均小于传统组,差异有统计学意义(P<0.05),而眼内高阶像差略低于传统组,但差异无统计学意义(P>0.05);飞白组术后眼内像差的慧差低于传统组,差异有统计学意义(P<0.05),三叶草像差比较差异无统计学意义(P>0.05)。结论 两种手术方式均可以减少白内障患者的全眼和眼内高阶像差,其中飞秒激光白内障手术对角膜高阶像差的影响较小,在降低眼内高阶像差方面更有优势,为术后更好的视觉质量提供保障。
Abstract:
Objective To compare the high order aberrations between femtosecond laser-assisted cataract surgery and traditional phacoemulsification surgery in cataract patients.Methods A total of 180 patients (180 eyes) who underwent cataract surgery in our hospital from January 2020 to March 2021 were selected and divided into two groups according to the random number table method, with 90 patients (90 eyes) in each group. The femtosecond laser-assisted cataract surgery (femtosecond laser group) and the traditional phacoemulsification surgery (traditional group) were performed respectively. The corneal, intraocular (coma and trefoil aberrations) and whole-eye higher-order aberrations were compared between the two groups before and after surgery.Results The postoperative high-order aberrations of the whole eye and intraocular in the two groups were lower than those before operation, and the difference was statistically significant (P<0.05). There was no significant difference in corneal high-order aberration between before and after operation in femtosecond laser group (P>0.05). The corneal high-order aberration in the traditional group was higher than that before operation, and the difference was statistically significant (P<0.05). The high-order aberrations of the whole eye and cornea in the femtosecond laser group were less than those in the traditional group, and the difference was statistically significant (P<0.05). The intraocular high-order aberrations in the femtosecond laser group were slightly lower than those in the traditional group, but the difference was not statistically significant (P>0.05). The coma of intra-ocular aberration in the femtosecond laser group was lower than that in the traditional group, and the difference was statistically significant (P<0.05). There was no statistically significant difference in the trefoil aberraition between femtosecond laser group and traditional group (P>0.05).Conclusion Both of the two surgical methods can reduce the high-order aberrations in the whole eye and eyes of cataract patients. Femtosecond laser-assisted cataract surgery has less influence on corneal high-order aberrations, and has more advantages in reducing intraocular high-order aberrations, which provides guarantee for better visual quality after surgery.
