摘要:
目的 观察经颅多普勒超声与头颅核磁检查对帕金森病患者的诊断价值。方法 选取2020年1月-2022年2月在佳木斯市肿瘤医院诊治的60例帕金森病患者为研究对象,定义为观察组,并选取同期在我院体检健康者60例为对照组,两组均进行经颅多普勒超声、头颅核磁检查。比较两组患者周围脑组织灰度值、黑质高回声面积、第三脑室宽度、脑桥面积、中脑面积、小脑中脚宽度、不同检测方法帕金森病(异常情况、轻度脑萎缩、腔隙性脑梗死、异常血管)检查结果以及诊断灵敏度、特异度。结果 观察组周围脑组织灰度值、黑质高回声面积均高于对照组(P<0.05);两组第三脑室宽度比较,差异无统计学意义(P>0.05);观察组脑桥面积、中脑面积、小脑中脚宽度与对照组比较,差异无统计学意义(P>0.05);经颅多普勒超声无异常情况、轻度脑萎缩、腔隙性脑梗死检出率与头颅核磁检出率比较,差异无统计学意义(P>0.05),异常血管检出率高于头颅核磁检出率(P<0.05);经颅多普勒超声联合头颅核磁诊断灵敏度高于经颅多普勒超声诊断、头颅核磁诊断,特异度低于经颅多普勒超声诊断、头颅核磁诊断,且经颅多普勒超声诊断灵敏度高于头颅核磁诊断(P<0.05),两者特异度比较,差异无统计学意义(P>0.05)。结论 经颅多普勒超声与头颅核磁检查对帕金森病患者均具有一定的诊断价值,相比较而言经颅多普勒超声诊断灵敏度高、操作简单,且特异性无显著差异。
Abstract:
Objective To observe the diagnostic value of transcranial Doppler ultrasound and cranial nuclear magnetic resonance in patients with Parkinson’s disease.Methods From January 2020 to February 2022,60 patients with Parkinson’s disease diagnosed and treated in Jiamusi Tumor Hospital were selected as the observation group, and 60 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. Both groups were examined by transcranial Doppler ultrasound and cranial nuclear magnetic resonance. The gray value of the surrounding brain tissue, the hyperechoic area of the substantia nigra, the width of the third ventricle, the area of the pons, the area of the midbrain, the width of the middle cerebellar peduncle, the results of different detection methods of Parkinson’s disease (abnormal condition, mild brain atrophy, lacunar infarction, abnormal blood vessels) and the diagnostic sensitivity and specificity were compared between the two groups.Results The gray value of the surrounding brain tissue and the hyperechoic area of the substantia nigra in the observation group were higher than those in the control group (P<0.05). There was no significant difference in the width of the third ventricle between the two groups (P>0.05). There was no significant difference in pons area, midbrain area and cerebellar peduncle width between the observation group and the control group (P>0.05). There was no significant difference in the detection rate of no abnormality, mild brain atrophy and lacunar infarction between transcranial Doppler ultrasound and cranial nuclear magnetic resonance (P>0.05). The detection rate of abnormal blood vessels by transcranial Doppler ultrasound was higher than that by cranial nuclear magnetic resonance (P<0.05). The diagnostic sensitivity of transcranial Doppler ultrasound combined with cranial nuclear magnetic resonance was higher than that of transcranial Doppler ultrasound and cranial nuclear magnetic resonance alone, and the specificity was lower than that of transcranial Doppler ultrasound and cranial nuclear magnetic resonance alone, and the diagnostic sensitivity of transcranial Doppler ultrasound was higher than that of cranial nuclear magnetic resonance (P<0.05). There was no significant difference in specificity between the two (P>0.05).Conclusion Transcranial Doppler ultrasound and cranial nuclear magnetic resonance examination have certain diagnostic value for patients with Parkinson’s disease. In comparison, transcranial Doppler ultrasound has high diagnostic sensitivity, simple operation, and no significant difference in specificity.
