摘要:
目的 研究核磁共振胰胆管成像联合腹部CT用于肝外胆管结石诊断的价值。方法 选取2019年5月-2022年5月在我院诊治的80例疑似肝外胆管结石患者为研究对象,均进行腹部CT检查、核磁共振胰胆管成像检查,以经内镜逆行性胰胆管造影术为金标准,比较腹部CT、核磁共振胰胆管成像以及腹部CT+核磁共振胰胆管成像诊断效能(准确率、灵敏度、特异度)、不同直径结石检出率、不同类型结石检出率。结果 腹部CT+核磁共振胰胆管成像诊断准确率、灵敏度、特异度均高于核磁共振胰胆管成像、腹部CT,且核磁共振胰胆管成像高于腹部CT(P<0.05);腹部CT+核磁共振胰胆管成像直径≤8 mm结石、直径>8 mm结石检出率均高于核磁共振胰胆管成像、腹部CT,核磁共振胰胆管成像直径≤8 mm结石检出率高于腹部CT(P<0.05);核磁共振胰胆管成像直径>8 mm结石检出率与腹部CT比较,差异无统计学意义(P>0.05);腹部CT+核磁共振胰胆管成像单纯肝内胆管结石检出率、胆囊合并胆管结石检出率与核磁共振胰胆管成像、腹部CT组间比较,差异无统计学意义(P>0.05);单纯胆管结石、单纯胆囊结石检出率均高于核磁共振胰胆管成像、腹部CT,且核磁共振胰胆管成像高于腹部CT(P<0.05)。结论 核磁共振胰胆管成像、腹部CT在肝外胆管结石诊断中均具有一定的价值,但是核磁共振胰胆管成像联合腹部CT诊断效能显著优于核磁共振胰胆管成像、腹部CT单一检查,且可准确判断结石直径、结石类型,为临床的诊治提供可靠参考。
Abstract:
Objective To study the value of nuclear magnetic resonance cholangiopancreatography combined with abdominal CT in the diagnosis of extrahepatic bile duct stones.Methods A total of 80 patients with suspected extrahepatic bile duct stones diagnosed and treated in our hospital from May 2019 to May 2022 were selected as the research objects. All patients underwent abdominal CT examination and nuclear magnetic resonance cholangiopancreatography. Endoscopic retrograde cholangiopancreatography was used as the gold standard to compare the diagnostic efficacy (accuracy, sensitivity and specificity) of abdominal CT, nuclear magnetic resonance cholangiopancreatography and abdominal CT + nuclear magnetic resonance cholangiopancreatography, as well as the detection rate of stones with different diameters and the detection rate of different types of stones.Results The diagnostic accuracy, sensitivity and specificity of abdominal CT+nuclear magnetic resonance cholangiopancreatography were higher than those of nuclear magnetic resonance cholangiopancreatography and abdominal CT, and nuclear magnetic resonance cholangiopancreatography was higher than abdominal CT (P<0.05). The detection rates of stones with diameter ≤8 mm and stones with diameter >8 mm by abdominal CT+nuclear magnetic resonance cholangiopancreatography were higher than those by nuclear magnetic resonance cholangiopancreatography and abdominal CT, the detection rate of stones with diameter≤8 mm by nuclear magnetic resonance cholangiopancreatography was higher than that by abdominal CT (P<0.05). There was no significant difference in the detection rate of stones with diameter >8 mm between nuclear magnetic resonance cholangiopancreatography and abdominal CT (P>0.05). There was no significant difference in the detection rate of simple intrahepatic bile duct stones and the detection rate of gallbladder combined with bile duct stones between abdominal CT+nuclear magnetic resonance cholangiopancreatography and nuclear magnetic resonance cholangiopancreatography and abdominal CT (P>0.05). The detection rates of simple bile duct stones and simple gallbladder stones were higher than those of nuclear magnetic resonance cholangiopancreatography and abdominal CT, and nuclear magnetic resonance cholangiopancreatography was higher than abdominal CT (P<0.05).Conclusion Nuclear magnetic resonance cholangiopancreatography and abdominal CT have certain value in the diagnosis of extrahepatic bile duct stones, but the diagnostic efficacy of nuclear magnetic resonance cholangiopancreatography combined with abdominal CT is significantly better than that of nuclear magnetic resonance cholangiopancreatography and abdominal CT alone, and can accurately determine the diameter and type of stones, providing a reliable reference for clinical diagnosis and treatment.
