目的 研究龈沟液中TIMP-1、MMP-8水平与广泛型侵袭性牙周炎患者牙槽嵴骨丢失的相关性。方法 选择2022年1月-4月在广丰区人民医院治疗的136例广泛型侵袭性牙周炎患者为研究对象,观察治疗前后患者龈沟液中MMP-8和TIMP-1水平、牙周健康指标[附着丧失(CAL)、探诊深度(PD)、出血指数(BI)、牙菌斑指数(PLI)]、牙槽嵴顶高度、牙槽嵴顶灰度值(HU),分析龈沟液中MMP-8和TIMP-1水平与以上各指标的相关性。结果 治疗3个月后,龈沟液中MMP-8低于治疗前,TIMP-1高于治疗前(P<0.05);治疗3个月后,患者CAL、PD、BI、PLI均低于治疗前(P<0.05);治疗3个月后,患者前磨牙区、磨牙区牙槽嵴顶高度大于治疗前(P<0.05),前牙区牙槽嵴顶高度与治疗前比较,差异无统计学意义(P>0.05);治疗3个月后,前磨牙区、磨牙区、前牙区牙槽骨HU值均高于治疗前(P<0.05);龈沟液MMP-8与CAL、PD、BI、PLI、牙槽嵴顶高度、HU值无相关性(P>0.05),TIMP-1与PLI呈负相关(P<0.05)。结论 广泛型侵袭性牙周炎患者龈沟液TIMP-1、MMP-8水平会一定程度升高,在治疗后龈沟液TIMP-1、MMP-8水平会下降,TIMP-1、MMP-8参与疾病的发生、进展,可判断疾病预后。同时TIMP-1水平与PLI呈负相关,临床可通过检测龈沟液中TIMP-1、MMP-8水平,了解牙槽嵴骨丢失情况,避免反复检查。
Abstract:
Objective To study the correlation between the levels of TIMP-1 and MMP-8 in gingival crevicular fluid and alveolar bone loss in patients with generalized aggressive periodontitis.Methods A total of 136 patients with generalized aggressive periodontitis who were treated in Guangfeng District People’s Hospital from January to April 2022 were selected as the study subjects. The levels of MMP-8 and TIMP-1 in gingival crevicular fluid, periodontal health indicators [clinical attachment level (CAL), probing depth (PD), bleeding index (BI), plaque index (PLI)], alveolar crest height, and alveolar crest gray value (HU) were observed before and after treatment. The correlation between MMP-8 and TIMP-1 levels in gingival crevicular fluid and the above indicators was analyzed.Results After 3 months of treatment, MMP-8 in gingival crevicular fluid was lower than that before treatment, and TIMP-1 was higher than that before treatment (P<0.05). After 3 months of treatment, the CAL, PD, BI and PLI of patients were lower than those before treatment (P<0.05). After 3 months of treatment, the height of alveolar crest in premolar area and molar area was higher than that before treatment (P<0.05), and there was no significant difference in the height of alveolar crest in anterior teeth area compared with that before treatment (P>0.05). After 3 months of treatment, the HU values of alveolar bone in premolar area, molar area and anterior teeth area were higher than those before treatment (P<0.05), and TIMP-1 was negatively correlated with PLI (P<0.05).Conclusion The levels of TIMP-1 and MMP-8 in gingival crevicular fluid of patients with generalized aggressive periodontitis will increase to a certain extent. After treatment, the levels of TIMP-1 and MMP-8 in gingival crevicular fluid will decrease. TIMP-1 and MMP-8 are involved in the occurrence and progression of the disease, which can judge the prognosis of the disease. At the same time, the level of TIMP-1 was negatively correlated with PLI. Clinically, the levels of TIMP-1 and MMP-8 in gingival crevicular fluid can be detected to understand the loss of alveolar bone and avoid repeated examination.
