摘要:
目的 研究口腔种植覆盖义齿(IOD)与全口义齿(CD)修复牙列缺失的临床疗效及其对牙周健康指标的影响。方法 选取2019年3月-2022年3月万载县人民医院口腔科收治的76例牙列缺失患者,采用随机数字表法分为IOD组(38例)与CD组(38例)。IOD组给予口腔种植覆盖义齿修复,CD组给予全口义齿修复,比较两组修复疗效、固位情况、牙周健康指标(牙龈指数、菌斑指数、槽嵴吸收情况)、咀嚼功能自评量表(CFQ)、口腔健康影响程度量表(OHIP-49)评分。结果 两组义齿修复后1个月咀嚼、发音、舒适度与美观度评分比较,差异无统计学意义(P>0.05);IOD组固位优良率为100.00%,高于CD组的89.47%(P<0.05);两组牙龈指数、菌斑指数比较,差异无统计学意义(P>0.05),但IOD组牙槽嵴吸收程度小于CD组(P<0.05);两组修复后3个月CFQ、OHIP-49评分均低于修复前,且IOD组CFQ、OHIP-49评分低于CD组(P<0.05)。结论 IOD与CD在牙列缺失的修复初期均具有良好疗效,其中IOD的固位效果更佳、牙槽骨吸收更少,对患者咀嚼功能及口腔健康的改善作用更为理想。
Abstract:
Objective To study the clinical effect of implant supported overdenture (IOD) and complete denture (CD) on dentition loss and their influence on periodontal health.Methods A total of 76 patients with dentition loss admitted to the Department of Stomatology, Wanzai County People’s Hospital from March 2019 to March 2022 were selected and divided into IOD group (38 patients) and CD group (38 patients) by random number table method. The IOD group was given oral implant supported overdenture repair, and the CD group was given complete denture repair. The repair efficacy, retention, periodontal health indicators (gingival index, plaque index, alveolar ridge absorption), chewing function self-rating scale (CFQ), and oral health impact profile scale (OHIP-49) scores were compared between the two groups.Results There was no significant difference in the scores of chewing, pronunciation, comfort and aesthetics between the two groups at 1 month after denture repair (P>0.05). The excellent and good rate of retention in the IOD group was 100.00%, which was higher than 89.47% in the CD group (P<0.05). There was no significant difference in gingival index and plaque index between the two groups (P>0.05), but the degree of alveolar ridge absorption in the IOD group was less than that in the CD group (P<0.05). The CFQ and OHIP-49 scores of the two groups at 3 months after repair were lower than those before repair, and the CFQ and OHIP-49 scores of the IOD group were lower than those of the CD group (P<0.05).Conclusion Both IOD and CD have good curative effect in the early stage of dentition loss repair. Among them, IOD has better retention effect, less alveolar bone absorption, and better improvement of masticatory function and oral health.
