摘要:
目的 总结脑肿瘤患者并发静脉血栓栓塞症的危险因素,以便临床医护人员及早识别疾病风险人群,为进行及时干预提供循证医学依据。方法 计算机检索The Cochrane Library、PubMed、EMbase、Web of Science、CINAHL、中国知网(CNKI)、万方、维普及生物医学数据库,收集自建库至2022年7月31日有关脑肿瘤患者并发静脉血栓栓塞症的研究,并对文献进行资料提取和质量评价,使用 RevMan 5.3软件进行Meta分析。结果 共纳入19篇文献,提取33项危险因素。Meta分析结果显示,高龄[OR=1.44,95%CI(1.31,1.59),P<0.000 01]、高级胶质瘤[OR=1.74,95%CI(1.47,2.06),P<0.000 01]、运动功能障碍[OR=1.92,95%CI(1.17,3.16),P=0.01]、体质指数[OR=1.13,95%CI(1.05,1.23),P=0.002]、手术时长>4 h[OR=1.66,95%CI(1.46,1.89),P<0.000 01]、术后感染[OR=7.87,95%CI(3.95,15.66),P<0.000 01]、呼吸机依赖[OR=2.76,95%CI(1.79,4.25),P<0.000 01]、D-二聚体>0.5 mg/L[OR=1.87,95%CI(1.43,2.45),P<0.000 01]、术中出血量多[OR=5.49,95%CI(1.69,17.86),P=0.005]是静脉血栓栓塞症的危险因素。结论 现有证据表明脑肿瘤患者发生静脉血栓栓塞症与高龄、高级胶质瘤、运动功能障碍、体质指数、手术时长>4 h、术后感染、呼吸机依赖、D-二聚体>0.5 mg/L、出血量多等多种因素相关,提示医务人员在临床工作中应识别相关危险因素,采取预防措施降低血栓风险。
Abstract:
Objective To summarize the risk factors of venous thromboembolism in patients with brain tumors, so that clinical medical staff can identify the risk population of the disease as early as possible, and provide evidence-based medical basis for timely intervention.Methods The Cochrane Library, PubMed, EMbase, Web of Science, CINAHL, China National Knowledge Infrastructure (CNKI), Wanfang, and Weipu Biomedical Database were searched by computer to collect studies on venous thromboembolism in patients with brain tumors from the establishment of the database to July 31, 2022.Data extraction and quality evaluation were performed on the literature, and Meta-analysis was performed using RevMan 5.3 software.Results A total of 19 articles were included, and 33 risk factors were extracted. The results of Meta-analysis showed that advanced age [OR=1.44, 95%CI(1.31, 1.59), P<0.000 01], advanced glioma [OR=1.74, 95%CI(1.47, 2.06), P<0.000 01], motor dysfunction[OR=1.92, 95%CI(1.17, 3.16), P=0.01], body mass index [OR=1.13, 95%CI(1.05,1.23), P=0.002], operation time >4 h[OR=1.66, 95%CI(1.46, 1.89),P<0.000 01], postoperative infection[OR=7.87, 95%CI(3.95, 15.66), P<0.000 01], ventilator dependence[OR=2.76, 95%CI(1.79, 4.25), P<0.000 01], D-dimer>0.5 mg/L[OR=1.87, 95%CI(1.43, 2.45), P<0.000 01], intraoperative blood loss [OR=5.49, 95%CI(1.69, 17.86), P=0.005]were risk factors for venous thromboembolism. Conclusion The existing evidence shows that the occurrence of venous thromboembolism in patients with brain tumors is related to many factors such as advanced age, advanced glioma, motor dysfunction, body mass index, operation time>4 h, postoperative infection, ventilator dependence, D-dimer>0.5 mg/L, and more bleeding. It is suggested that medical staff should identify relevant risk factors in clinical work and take preventive measures to reduce the risk of thrombosis.
