摘要:
目的 分析抗VEGF药物联合激光光凝术治疗视网膜分支静脉阻塞前后视网膜血管的变化。方法 选取2021年3月-2023年1月我院诊治的70例视网膜分支静脉阻塞患者作为观察组,另选取同期我院体检健康者70名作为对照组,观察组采用抗VEGF药物联合激光治疗,比较两组视网膜动脉和静脉血管指标[血管壁厚度(WT)、血管壁截面面积(WCSA)、壁腔比(WLR)];并分析观察组治疗前后视网膜静脉血管指标(WT、WCSA、WLR)、最佳矫正视力(BCVA)、中心视网膜厚度(CRT)及并发症发生率。结果 观察组视网膜动脉WT、WCSA、WLR与对照组比较,差异无统计学意义(P>0.05),而观察组视网膜静脉WT、WCSA、WLR均大于对照组(P<0.05);观察组治疗3个月后WT、WCSA、WLR及CRT均小于治疗前,BCVA大于治疗前(P<0.05);且观察组患者均无眼内炎、玻璃体积血、视网膜脱离等严重并发症,仅出现轻度结膜下出血,未进行干预后自行吸收;眼压升高患者给予针对性干预后均恢复正常。结论 视网膜分支静脉阻塞患者视网膜血管管径狭窄,血管壁增厚,临床可将WT、WCSA、WLR指标作为预测视网膜分支静脉阻塞的重要参数指标,且抗VEGF药物联合激光光凝术治疗视网膜分支静脉阻塞的效果良好,可促进视网膜血管指标恢复。
Abstract:
Objective To analyze the changes of retinal blood vessels before and after anti-VEGF drugs combined with laser photocoagulation in the treatment of branch retinal vein occlusion.Methods A total of 70 patients with branch retinal vein occlusion diagnosed and treated in our hospital from March 2021 to January 2023 were selected as the observation group, and 70 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. The observation group was treated with anti-VEGF drugs combined with laser. The retinal artery and vein vascular indexes [vascular wall thickness (WT), vascular wall cross-sectional area (WCSA), wall-to-cavity ratio (WLR)] were compared between the two groups. The retinal vein vascular indexes (WT, WCSA, WLR), best corrected visual acuity (BCVA), central retinal thickness (CRT) and the incidence of complications before and after treatment in the observation group were analyzed.Results There was no significant difference in WT, WCSA and WLR of retinal artery between the observation group and the control group (P>0.05), while WT, WCSA and WLR of retinal vein in the observation group were higher than those in the control group (P<0.05). After 3 months of treatment, WT, WCSA, WLR and CRT in the observation group were lower than those before treatment, and BCVA was higher than that before treatment (P<0.05). There were no serious complications such as endophthalmitis, vitreous hemorrhage and retinal detachment in the observation group, and only mild subconjunctival hemorrhage occurred, which was absorbed by itself without intervention; patients with elevated intraocular pressure returned to normal after targeted intervention.Conclusion The diameter of retinal vessels in patients with branch retinal vein occlusion is narrow and the vascular wall is thickened. WT, WCSA and WLR can be used as important parameters to predict branch retinal vein occlusion in clinic. Anti-VEGF drugs combined with laser photocoagulation are effective in the treatment of branch retinal vein occlusion, which can promote the recovery of retinal vascular indexes.
