摘要:
目的 研究瑞芬太尼复合环泊酚静注麻醉在腹腔镜胆囊切除术中的应用效果。方法 选取2021年1月-2023年7月我院行腹腔镜胆囊切除术的60例患者为研究对象，采用随机数字表法分为观察组（n=30）和对照组（n=30），对照组采用瑞芬太尼麻醉，观察组在对照组基础上复合环泊酚静注麻醉，比较两组麻醉效果、麻醉指标（自主呼吸恢复时间、意识恢复时间、苏醒期镇静评分）、不同时间点疼痛（VAS）评分、应激反应指标[平均动脉压（MAP）、心率（HR）]、不良反应发生情况。结果 观察组麻醉优良率为93.33%，高于对照组的80.00%（P＜0.05）；观察组自主呼吸恢复时间、意识恢复时间均短于对照组，苏醒期镇静评分低于对照组（P＜0.05）；观察组术后6、12、24 h的VAS评分均低于对照组（P＜0.05）；观察组麻醉后MAP、HR均高于对照组（P＜0.05）；观察组不良反应发生率为6.67%，低于对照组的23.33%（P＜0.05）。结论 瑞芬太尼复合环泊酚静注麻醉在腹腔镜胆囊切除术中的应用效果理想，可降低麻醉不良反应、减小应激反应、减轻患者疼痛度、缩短麻醉恢复指标，是一种可行的麻醉方案，值得临床加以应用。
Abstract:
Objective To study the application effect of remifentanil combined with propofol intravenous anesthesia in laparoscopic cholecystectomy.Methods A total of 60 patients who underwent laparoscopic cholecystectomy in our hospital from January 2021 to July 2023 were selected as the research objects. They were divided into observation group (n=30) and control group (n=30) by random number table method. The control group was anesthetized with remifentanil, and the observation group was combined with propofol intravenous anesthesia on the basis of the control group. The anesthesia effect, anesthesia index (spontaneous breathing recovery time, consciousness recovery time, recovery period sedation score), pain (VAS) score at different time points, stress response index [mean arterial pressure (MAP), heart rate (HR)] and adverse reactions were compared between the two groups.Results The excellent and good rate of anesthesia in the observation group was 93.33%, which was higher than 80.00% in the control group (P<0.05). The spontaneous breathing recovery time and consciousness recovery time in the observation group were shorter than those in the control group, and the sedation score in the recovery period was lower than that in the control group (P<0.05). The VAS score of the observation group at 6, 12 and 24 h after operation was lower than that of the control group (P<0.05). After anesthesia, MAP and HR in the observation group were higher than those in the control group (P<0.05). The incidence of adverse reactions in the observation group was 6.67%, which was lower than 23.33% in the control group (P<0.05).Conclusion The application effect of remifentanil combined with propofol intravenous anesthesia in laparoscopic cholecystectomy is ideal, and it can reduce the adverse reactions of anesthesia, the stress response and the pain, as well as shorten the recovery index of anesthesia. It is a feasible anesthesia scheme and worthy of clinical application.
