摘要:
目的 分析无创呼吸机联合安全护理对慢阻肺合并Ⅱ型呼吸衰竭患者的影响。方法 选取2021年8月-2022年6月在我院诊治的104例慢阻肺合并Ⅱ型呼吸衰竭患者为研究对象，采用随机数字表法分为对照组和观察组，各52例。两组患者均采用无创呼吸机治疗，对照组给予常规护理，观察组在对照组基础上给予安全护理，比较两组血气指标、肺功能指标、护理满意度，以及并发症发生率。结果 两组干预后动脉血二氧化碳分压（PaCO2）均低于干预前，血氧分压（PaO2）、血氧饱和度（SaO2）均高于干预前，且观察组PaCO2均低于对照组，PaO2、SaO2高于对照组（P＜0.05）；两组干预后1秒用力呼气容积（FEV1）、用力肺活量（FVC）、FEV1/FEV比值（FEV1%）均高于干预前，且观察组高于对照组（P＜0.05）；观察组护理满意度高于对照组（P＜0.05）；观察组并发症发生率为3.85%，低于对照组的11.54%（P＜0.05）。结论 无创呼吸机联合安全护理对慢阻肺合并Ⅱ型呼吸衰竭患者康复具有积极的影响，可提高患者护理满意度，改善肺功能和血气指标，预防并发症的发生，是一种有效的护理干预模式，值得临床应用。
Abstract:
Objective To analyze the effect of non-invasive ventilator combined with safety nursing on patients with chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure.Methods A total of 104 patients with chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure who were treated in our hospital from August 2021 to June 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 52 patients in each group. The two groups of patients were treated with non-invasive ventilator, the control group was given routine nursing, and the observation group was given safe nursing on the basis of the control group. The blood gas index, lung function index, nursing satisfaction and complication rate were compared between the two groups.Results After intervention, the arterial partial pressure of carbon dioxide (PaCO2) in the two groups was lower than that before intervention, and the partial pressure of oxygen (PaO2) and blood oxygen saturation (SaO2) were higher than those before intervention, and the PaCO2 in the observation group was lower than that in the control group, and PaO2 and SaO2 were higher than those in the control group(P<0.05). The forced expiratory volume in one second (FEV1), forced vital capacity (FVC) and FEV1/FEV ratio (FEV1%) in the two groups after intervention were higher than those before intervention, and those in the observation group were higher than those in the control group (P<0.05). The nursing satisfaction of the observation group was higher than that of the control group (P<0.05). The incidence of complications in the observation group was 3.85%, which was lower than 11.54% in the control group (P<0.05).Conclusion Non-invasive ventilator combined with safety nursing has a positive effect on the rehabilitation of patients with chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure, which can improve patients’ nursing satisfaction, improve lung function and blood gas index, and prevent complications. It is an effective nursing intervention mode and worthy of clinical application.
