摘要:
目的 研究3D Slicer软件辅助定位在高血压脑出血微创钻孔引流术中的效果。方法 选取2020年6月-2023年5月赣南医学院第三附属医院行微创钻孔引流手术的60例高血压脑出血患者为研究对象,采用随机数字表法分为对照组和观察组,各组30例。对照组采用单纯CT方法定位,观察组采用3D Slicer辅助定位穿刺,比较两组临床手术指标(手术时间、血肿清除率、术中出血量)、格拉斯哥昏迷评分(GCS)、日常生活活动能力评分(ADL)、格拉斯哥预后评分(GOS)、神经功能缺损评分(NIHSS)、并发症发生率。结果 观察组手术时间短于对照组,术中出血量少于对照组,血肿清除率大于对照组(P<0.05);两组术后GCS评分、ADL评分、GOS评分高于术前,NIHSS评分低于术前,且观察组GCS评分、ADL评分、GOS评分高于对照组,NIHSS评分低于对照组(P<0.05);观察组并发症发生率为6.67%,低于对照组的20.00%(P<0.05)。结论 利用3D Slicer软件辅助定位在高血压脑出血微创钻孔引流手术治疗中的效果理想,可降低并发症发生率,促进神经功能缺损恢复,改善患者日常生活能力和手术指标,利于良好预后的形成。
Abstract:
Objective To study the effect of 3D Slicer software-assisted positioning in minimally invasive drilling and drainage for hypertensive cerebral hemorrhage.Methods A total of 60 patients with hypertensive cerebral hemorrhage who underwent minimally invasive drilling and drainage in the Third Affiliated Hospital of Gannan Medical University from June 2020 to May 2023 were selected as the research objects. They were divided into control group and observation group by random number table method, with 30 patients in each group. The control group was positioned by simple CT method, and the observation group was positioned by 3D Slicer assisted puncture. The clinical operation indexes (operation time, hematoma clearance rate, intraoperative blood loss), Glasgow Coma Score (GCS), activity of daily living score (ADL), Glasgow Prognosis Score (GOS), neurological deficit score (NIHSS) and complication rate were compared between the two groups.Results The operation time of the observation group was shorter than that of the control group, the intraoperative blood loss was less than that of the control group, and the hematoma clearance rate was greater than that of the control group (P<0.05). The GCS score, ADL score and GOS score of the two groups after operation were higher than those before operation, the NIHSS score was lower than that before operation, and the GCS score, ADL score and GOS score of the observation group were higher than those of the control group, the NIHSS score was lower than that of the control group (P<0.05). The incidence of complications in the observation group was 6.67%, which was lower than 20.00% in the control group (P<0.05).Conclusion The effect of 3D Slicer software-assisted positioning in the treatment of minimally invasive drilling and drainage for hypertensive cerebral hemorrhage is ideal, which can reduce the incidence of complications, promote the recovery of neurological deficits, improve the daily living ability and surgical indicators of patients, and is conducive to the formation of good prognosis.
