目的 探讨磁共振mDixon Quant、T2 mapping定量技术评估慢性下腰痛椎旁肌群和椎间盘退变的应用价值。方法 收集2021年7月-2022年8月于我院就诊的54例慢性腰痛患者作为腰痛组,另选取50名健康志愿者作为对照组,两组均采用矢状位T2 mapping、轴位mDixon Quant序列和常规腰椎序列进行扫描,测量腰大肌(PM)、竖棘肌(ES)的脂肪分数(FF)和前纤维环(AAF)、髓核(NP)、后纤维环(PAF)的T2值。比较腰痛组与对照组PM、ES脂肪分数及椎间盘AAF、NP、PAF T2值的差异,分析腰痛组椎旁肌脂肪分数与椎间盘退变的相关性。结果 腰痛组与对照组NP的T2值比较,差异有统计学意义(P<0.05),两组PM、ES脂肪分数、椎间盘AAF、PAF的T2值比较,差异无统计学意义(P>0.05)。腰痛组竖棘肌脂肪分数与椎间盘退变呈中度正相关(r=0.570,P<0.05)。结论 慢性腰痛患者竖棘肌脂肪分数与腰椎间盘退变之间存在中等程度相关性,尤其与髓核区退变相关,因此椎旁肌脂肪分数可作为评估椎间盘退变的生物标志物之一。
Abstract:
Objective To explore the application value of magnetic resonance mDixon Quant and T2 mapping quantitative techniques in evaluating paravertebral muscle group and intervertebral disc degeneration in chronic low back pain.Methods A total of 54 patients with chronic low back pain who were treated in our hospital from July 2021 to August 2022 were collected as the low back pain group, and 50 healthy volunteers were selected as the control group. Both groups were scanned by sagittal T2 mapping, axial mDixon Quant sequence and conventional lumbar sequence. The fat fraction (FF) of psoas major (PM) and erector spinae (ES) and the T2 values of anterior annulus fibrosus (AAF), nucleus pulposus (NP) and posterior annulus fibrosus (PAF) were measured. The differences of PM, ES fat fraction and intervertebral disc AAF, NP, PAF T2 values between the low back pain group and the control group were compared, and the correlation between the fat fraction of paraspinal muscle and intervertebral disc degeneration in the low back pain group was analyzed.Results There was significant difference in T2 value of NP between the low back pain group and the control group (P<0.05), but there were no significant differences in the T2 values of PM, ES fat fraction, intervertebral disc AAF and PAF between the two groups (P>0.05). There was a moderate positive correlation between the fat fraction of erector spinae and intervertebral disc degeneration in the low back pain group (r=0.570, P<0.05).Conclusion There is a moderate correlation between the fat fraction of erector spinae muscle and lumbar disc degeneration in patients with chronic low back pain, especially in the nucleus pulposus area. Therefore, the fat fraction of paraspinal muscle can be used as one of the biomarkers for evaluating intervertebral disc degeneration.
