目的 比较我国新疆喀什地区结核(TB)患者、人类免疫缺陷病毒(HIV)感染者及艾滋病合并结核共感染(HIV-TB)共感染者外周血外周血单个核细胞(PBMC)中CD4+T细胞、Th17细胞及Treg细胞水平,及其相关细胞因子的表达水平。方法 收集2021年5月-2022年12月就诊于喀什地区第一人民医院的初治TB患者24例、HIV患者21例、HIV-TB患者17例。采用流式细胞术检测各患者PBMCs中CD4+T细胞、Th17、Treg细胞水平及Th17/Treg比例;采用酶联免疫吸附(ELISA)方法检测各组患者血清中IL-2、IL-10、IL-17、IFN-γ及TNF-α等细胞因子的表达水平。结果 与TB、HIV患者相比,HIV-TB患者CD4+T、Th17细胞水平下调(P<0.05),Treg细胞水平上调(P<0.05),Th17/Treg比例下调(P<0.05)。与TB患者相比,HIV-TB患者IL-2、IFN-γ及IL-17浓度降低,IL-10、TNF-α浓度升高(P<0.05);与HIV患者相比,HIV-TB患者IL-2、IL-17浓度降低,IL-10、IFN-γ和TNF-α浓度升高(P<0.05)。结论 与TB和HIV患者相比,HIV-TB患者Th17/Treg免疫轴的失衡更为显著。与TB和HIV患者相比,HIV-TB患者免疫水平异常更加严重。
Abstract:
Objective To compare the levels of CD4+ T cells, Th17 cells and Treg cells in peripheral blood mononuclear cells (PBMC) of patients with tuberculosis (TB), human immunodeficiency virus (HIV) infection and AIDS complicated with tuberculosis(HIV-TB) co-infection in Kashgar, Xinjiang, China, and the expression levels of related cytokines. Methods From May 2021 to December 2022, 24 patients with newly diagnosed TB, 21 patients with HIV and 17 patients with HIV-TB were collected from the First People’s Hospital of Kashgar. Flow cytometry was used to detect the levels of CD4+ T cells, Th17, Treg cells and Th17/Treg ratio in PBMCs of each patient. Enzyme-linked immunosorbent assay (ELISA) was used to detect the expression levels of IL-2, IL-10, IL-17, IFN-γ and TNF-α in the serum of each patient. Results Compared with TB and HIV patients, the levels of CD4+ T and Th17 cells in HIV-TB patients were down-regulated (P<0.05), the level of Treg cells was up-regulated (P<0.05), and the ratio of Th17/Treg was down-regulated (P<0.05). Compared with TB patients, the concentrations of IL-2, IFN-γ and IL-17 in HIV-TB patients were decreased, and the concentrations of IL-10 and TNF-α were increased (P<0.05). Compared with HIV patients, the concentrations of IL-2 and IL-17 in HIV-TB patients decreased, and the concentrations of IL-10, IFN-γ and TNF-α increased (P<0.05). Conclusion Compared with TB and HIV patients, the imbalance of Th17/Treg immune axis in HIV-TB patients is more significant. Compared with TB and HIV patients, the abnormal immune level of HIV-TB patients is more serious.
