摘要:
目的 分析自贡地区2021-2023年人乳头瘤病毒(HPV)感染情况、亚型分布特征,以及行阴道镜和宫颈组织病理学检查HPV阳性患者的病理组织活检结局。方法 回顾性分析2021年1月-2023年9月在自贡市第四人民医院健康管理科、妇科门诊就诊的42 615例女性患者HPV感染状况,按年龄分为<25岁组(n=1096)、25~34岁组(n=10 127)、35~44岁组(n=11 831)、45~54岁组(n=14 013)、≥55岁组(n=5548),采集宫颈脱落细胞,HPV分型检测采用液态基因芯片及PCR法,可检测27种HPV基因分型。用构成比分析不同年龄段患者HPV亚型分布情况;采用皮尔逊相关分析法分析178例HPV阳性同时进行阴道镜和宫颈组织病理学检查的患者HPV各亚型感染情况与病理组织活检结局的关系。结果 HPV总体阳性率为16.00%。在所有HPV阳性患者中,HPV单一型占73.50%,HPV多重型占26.50%。占比从高到低前6位的HPV型别依次为52型(25.24%)、53型(13.74%)、58型(12.51%)、16型(11.91%)、61型(10.30%)、81型(9.52%)。各年龄段组间总体感染率比较,差异有统计学意义(P<0.05),其中<25岁组HPV感染率最高,为29.01%,高于25~34岁组(16.96%)、35~44岁组(13.68%)、45~54岁组(15.38%)、≥55岁组(18.15%),差异均有统计学意义(P均<0.05)。HPV16在宫颈上皮内瘤变(CIN)3级及宫颈癌患者中阳性率为71.43%,高于炎性改变的病例(25.23%)和CIN1级的病例(23.68%),差异均有统计学意义(P<0.05)。结论 <25岁人群HPV感染率最高,全部就诊人群中,高危型HPV52型、53型及58型是自贡地区女性最常见的感染型别,但高级别宫颈上皮内瘤变及宫颈癌与HPV16型感染更为密切。
Abstract:
Objective To analyze the infection status and subtype distribution characteristics of human papillomavirus (HPV) in Zigong area from 2021 to 2023, as well as the pathological tissue biopsy results of HPV positive patients undergoing colposcopy and cervical histopathological examination. Methods A retrospective analysis was conducted on the HPV infection status of 42 615 female patients who visited the Health Management Department and Gynecology Clinic of the Zigong Fourth People’s Hospital from January 2021 to September 2023. They were divided into the <25-year-old group (1096 patients), the 25-34-year-old group (10 127 patients), the 35-44-year-old group (11 831 patients), the 45-54-year-old group (14 013 patients), and the ≥55-year-old group (5548 patients). Cervical exfoliated cells were collected. HPV typing was detected by liquid gene chip and PCR, and 27 HPV genotypes were detected. The distribution of HPV subtypes in patients of different ages was analyzed by composition ratio. Pearson correlation analysis was used to analyze the relationship between HPV subtypes infection and pathological biopsy outcome in 178 patients with HPV positive who underwent colposcopy and cervical histopathological examination at the same time. Results The overall positive rate of HPV was 16.00%. Among all HPV positive patients, HPV monotype accounted for 73.50%, and HPV multi type accounted for 26.50%. The top 6 HPV types from high to low were 52 (25.24%), 53 (13.74%), 58 (12.51%), 16 (11.91%), 61 (10.30%) and 81 (9.52%). There was a statistically significant difference in the overall infection rate among different age groups (P<0.05). The HPV infection rate in the <25-year-old group was the highest (29.01%), which was higher than that in the 25-34-year-old group (16.96%), the 35-44-year-old group (13.68%), the 45-54-year-old group (15.38%), and the ≥55-year-old group (18.15%), the differences was statistically significant (all P<0.05). The positive rate of HPV16 in patients with cervical intraepithelial neoplasia (CIN) grade 3 and cervical cancer was 71.43%, which was higher than that in patients with inflammatory changes (25.23%) and CIN grade 1 (23.68%), and the difference was statistically significant (P<0.05). Conclusion The population under 25 years old has the highest HPV infection rate. Among the entire population, high-risk HPV 52, 53, and 58 types are the most common infection types among women in Zigong area, but high-level cervical intraepithelial neoplasia and cervical cancer are more closely associated with HPV16 infection.
