摘要:
目的 应用更符合中国儿科特征的营养筛查工具评估住院儿童的营养不良风险,并为该工具在中国住院患儿中的推广及合理应用提供临床依据。方法 选取2023年6月-9月甘肃省妇幼保健院/甘肃省中心医院儿内、儿外及PICU的住院患儿作为研究对象,运用儿科营养筛查评分(PNSS)进行营养筛查,比较不同营养风险患儿的临床结局。结果 32.50%的住院患儿存在营养不良风险。不同科室营养不良风险发生率比较,差异有统计学意义(P<0.05),其中PICU(59.19%)、小儿消化科(57.61%)、心脏大血管外科(51.43%)存在营养不良风险的患儿比例较高。不同年龄组住院患儿的营养不良风险比较,差异有统计学意义(P<0.05),其中0~1岁组患儿的营养不良风险发生率为39.79%,高于其他年龄组;有营养不良风险的患儿住院天数及住院费用均高于无营养不良风险的患儿,且其疾病好转率低于无营养不良风险的患儿,差异有统计学意义(P<0.05)。结论 新型儿科营养筛查工具能有效识别住院患儿发生营养不良的风险并预测临床结局,适用于临床实践。
Abstract:
Objective To assess the risk of malnutrition in hospitalized children using a nutritional screening tool that meets the characteristics of pediatrics in China, and to provide clinical evidence for the promotion and rational application of this tool in hospitalized children in China. Methods From June to September 2023, hospitalized children in Gansu Provincial Maternity and Child-care Hospital/Gansu Provincial Central Hospital and PICU were selected as the research objects. Pediatric Nutrition Screening Score (PNSS) was used for nutritional screening, and the clinical outcomes of children with different nutritional risks were compared. Results Totally 32.50% of the hospitalized children had the risk of malnutrition. There was a statistically significant difference in the incidence of malnutrition risk among different departments (P<0.05), among them, PICU (59.19%), pediatric digestive department (57.61%), and cardiac and vascular surgery (51.43%) had a higher proportion of children with malnutrition risk. There was a statistically significant difference in the risk of malnutrition among hospitalized children with different age groups (P<0.05); the incidence of malnutrition risk in children aged 0-1 years was 39.79%, which was higher than that in other age groups; the hospitalization days and hospitalization expenses of children with malnutrition risk were higher than those of children without malnutrition risk, and the disease improvement rate was lower than that of children without malnutrition risk, the difference was statistically significant (P<0.05). Conclusion The new pediatric nutrition screening tool can effectively identify the risk of malnutrition in hospitalized children and predict clinical outcomes, which is suitable for clinical practice.
