摘要:
目的 探究痔疮自动套扎术（RPH）对直肠前突所致出口梗阻型便秘（OOC）患者临床疗效和心理状况的影响。方法 选取2021年1月-2023年11月赣县区人民医院收治的76例OOC患者，经随机数字表法分为对照组（38例）与研究组（38例）。对照组行经直肠切开前突修补术治疗，研究组则采用RPH术治疗，对两组临床疗效、手术相关指标（手术时间、术后首次排便时间、术后住院时间、创面痊愈时间）、排便情况、心理状况[焦虑自评量表（SAS）]、术后并发症发生情况进行比较。结果 与对照组临床总有效率的78.95%比较，研究组临床总有效率更高，达94.74%（P＜0.05）；与对照组比较，研究组手术时间、术后首次排便时间、术后住院时间、创面痊愈时间更短（P＜0.05）；与术前比较，两组术后排便情况、SAS评分改善，且与对照组比较，研究组术后排便情况、SAS评分更优（P＜0.05）；与对照组术后并发症发生率的23.68%比较，研究组术后并发症发生率更低，为5.26%（P＜0.05）。结论 RPH术对直肠阴道前突所致OOC患者具有确切治疗作用，其手术时间短、术后恢复快、术后并发症少，可有效改善患者排便功能，缓解其不良情绪。
Abstract:
Objective To explore the effect of Ruiyun procedure for hemorrhoids (RPH) on the clinical efficacy and psychological status of patients with outlet obstructive constipation (OOC) caused by rectocele. Methods A total of 76 patients with OOC admitted to Ganxian District People’s Hospital from January 2021 to November 2023 were selected and divided into control group (38 patients) and study group (38 patients) by random number table method. The control group was treated with transrectal incision and protrusion repair, while the study group was treated with RPH. The clinical efficacy, operation related indexes (operation time, first defecation time after operation, hospitalization time after operation, wound healing time), defecation, psychological status [Self-rating Anxiety Scale (SAS)] and postoperative complications were compared between the two groups. Results Compared with the control group, the total clinical effective rate was 78.95%, and the total clinical effective rate of the study group was higher, reaching 94.74% (P<0.05). Compared with the control group, the operation time, postoperative first defecation time, postoperative hospitalization time and wound healing time of the study group were shorter (P<0.05). Compared with before operation, the postoperative defecation and SAS scores of the two groups had improved, and compared with the control group, the postoperative defecation and SAS scores of the study group were better (P<0.05). Compared with 23.68% of the control group, the incidence of postoperative complications in the study group was lower, which was 5.26% (P<0.05). Conclusion RPH has a definite therapeutic effect on OOC patients caused by rectovaginal protrusion. It has short operation time, rapid postoperative recovery and few postoperative complications. Meanwhile, it can effectively improve the defecation function of patients and alleviate their bad mood.
