摘要:
目的 探讨口面肌-膈肌功能训练对老年轻中度阻塞性睡眠呼吸暂停(OSA)患者的OSA严重程度、临床症状、睡眠质量及生活质量等方面的治疗效果。方法 选择2021年6月-2022年12月首次就诊于新疆维吾尔自治区人民医院完善多导睡眠监测明确诊断为轻中度OSA的60~80岁老年患者50例,随机分成对照组和口面肌-膈肌功能训练(ODMT)组,每组25例,对照组予以深呼吸训练,ODMT组予以ODMT。比较两组患者基线时和治疗8周后的呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)、Epworth嗜睡量表(ESS)、匹兹堡睡眠质量指数(PSQI)、柏林问卷(BQ)、健康调查简表(SF-36)评分。结果 基线时两组患者AHI、LSaO2、ESS、PSQI、BQ、SF-36评分比较,差异无统计学意义(P>0.05)。治疗8周后,ODMT组AHI、ESS评分、BQ评分、PSQI评分低于对照组,SF-36评分中生理机能、生理职能、精力评分高于对照组(P<0.05)。与基线相比,治疗8周后ODMT组AHI、ESS评分、BQ评分、PSQI评分降低,SF-36评分升高(P<0.05)。多元线性回归分析显示,ODMT是AHI、ESS、PSQI、BQ评分下降及SF-36评分中生理机能、生理职能、躯体疼痛、精力、精神健康评分升高的独立影响因素(P<0.05)。结论 口面肌-膈肌功能训练可有效地改善老年轻中度阻塞性睡眠呼吸暂停的AHI、白天嗜睡、打鼾、睡眠质量及生活质量。
Abstract:
Objective To investigate the effects of the orofacial-diaphragmatic myofunctional training on OSA severity, clinical symptoms, sleep quality and quality of life in elderly patients with mild to moderate obstructive sleep apnea (OSA). Methods From June 2021 to December 2022, 50 elderly patients aged 60-80 years who were first diagnosed as mild to moderate OSA by polysomnography in the People’s Hospital of Xinjiang Uygur Autonomous Region were randomly divided into control group and orofacial-diaphragmatic myofunctional training (ODMT) group, with 25 patients in each group. The control group was given deep breathing training, and the ODMT group was given ODMT. The apnea hypopnea index (AHI), lowest arterial oxygen saturation (LSaO2), epworth sleepiness scale (ESS), Pittsburgh sleep quality index (PSQI), Berlin questionnaire (BQ) and 36-item short form survey (SF-36) were compared between the two groups at baseline and 8 weeks after treatment. Results There was no significant difference in AHI, LSaO2, ESS, PSQI, BQ and SF-36 scores between the two groups at baseline (P>0.05). After 8 weeks of treatment, the scores of AHI, ESS, BQ and PSQI in the ODMT group were lower than those in the control group, and the scores of physiological function, physiological function and energy in the SF-36 score were higher than those in the control group (P<0.05). Compared with baseline, AHI, ESS score, BQ score and PSQI score decreased and SF-36 score increased in ODMT group after 8 weeks of treatment (P<0.05). Multivariate linear regression analysis showed that ODMT was an independent influencing factor for the decrease of AHI, ESS, PSQI and BQ scores and the increase of physiological function, physiological function, physical pain, energy and mental health scores in SF-36 score (P<0.05). Conclusion Orofacial-diaphragmatic myofunctional training can effectively improve AHI, daytime sleepiness, snoring, sleep quality and quality of life in elderly patients with mild to moderate obstructive sleep apnea.
