摘要:
目的 分析使用动态张口位结合比值法诊断寰枢关节失稳、半脱位的应用效果。方法 选取永丰县人民医院2021年11月-2023年12月收治的疑似寰枢关节失稳、半脱位患者60例作为研究对象，分别予以动态张口位结合比值法诊断，金标准为MSCT图像结果，分析患者寰枢关节间隙比值及活动后间隙改变情况，观察动态张口位、比值法以及联合检查的敏感度、特异度、准确率，评估诊断精准性。结果 60例患者均获得满意的图像质量，结合比值法诊断为环枢关节正常的9例，可疑失稳8例，失稳23例，半脱位15例，脱位5例。动态张口位结合比值法诊断的灵敏度、特异度、准确性高于动态张口位、比值法单独检查，差异有统计学意义（P＜0.05）。结论 采用动态张口位结合比值法能够比较全面而准确地对寰枢关节失稳、半脱位进行早期诊断，为临床治疗提供有力的影像学支持。
Abstract:
Objective To analyze the effect of dynamic mouth opening position combined with ratio method in the diagnosis of atlantoaxial joint instability and subluxation. Methods A total of 60 patients with suspected atlantoaxial joint instability and subluxation admitted to Yongfeng County People’s Hospital from November 2021 to December 2023 were selected as the study objects. All patients were diagnosed by dynamic mouth opening position combined with ratio method. The gold standard was the result of MSCT image. The ratio of atlantoaxial joint space and the change of space after activity were analyzed. The sensitivity, specificity and accuracy of dynamic mouth opening, ratio method and combined examination were observed to evaluate the accuracy of diagnosis. Results The image quality of 60 patients was satisfactory, 9 patients were diagnosed as normal, 8 as suspected instability, 23 as unstable, 15 as subluxation, and 5 as dislocation by ratio method. The sensitivity, specificity and accuracy of dynamic mouth opening position combined with ratio method were higher than that of dynamic mouth opening position and ratio method alone, and the difference was statistically significant (P<0.05). Conclusion Dynamic mouth opening position combined with ratio method can diagnose atlantoaxial joint instability and subluxation comprehensively and accurately, and provide strong imaging support for clinical treatment.
