摘要:
目的 观察新活素与沙库巴曲缬沙坦联合治疗对顽固性心衰患者左室射血分数及心血管不良事件的影响。方法 选取徐州市铜山区人民医院2021年10月-2024年10月接收的顽固性心衰患者70例作为研究对象，遵循随机数字表法分为对照组35例和观察组35例，其中对照组应用单纯沙库巴曲缬沙坦治疗，观察组采取沙库巴曲缬沙坦+新活素治疗。比较两组临床疗效、2个月内再入院率、血清指标[N末端脑钠肽前体（NT-proBNP）、可溶性人基质裂解素2（sST2）、高敏C反应蛋白（hs-CRP）、肌钙蛋白Ⅰ（TnⅠ）]、肾功能指标[尿素氮、血肌酐、肾小球滤过率（eGFR）、尿量]及心超指标[左室舒张末期内径（LVEDD）、左室收缩末期内径（LVESD）、左室射血分数（LVEF）]。结果 观察组总有效率（94.29%）高于对照组（68.57%），2个月内再入院率（8.57%）低于对照组（31.43%）（P＜0.05）。治疗后，观察组NT-proBNP、sST2、hsCRP、TnⅠ均低于对照组（P＜0.05）。治疗后，观察组尿素氮、血肌酐水平低于对照组，eGFR、尿量高于对照组，LVEDD、LVESD小于对照组，LVEF大于对照组（P＜0.05）。结论 新活素与沙库巴曲缬沙坦联合治疗可改善顽固性心衰患者的心功能，降低血清炎症及心肌损伤标志物水平，保护肾功能，值得临床应用。
Abstract:
Objective To observe the effect of rhBNP combined with sacubitril valsartan on left ventricular ejection fraction and cardiovascular adverse events in patients with refractory heart failure. Methods A total of 70 patients with refractory heart failure received in Tongshan People′s Hospital of Xuzhou from October 2021 to October 2024 were selected as the research objects. According to the random number table method, they were divided into control group (n=35) and observation group (n=35). The control group was treated with sacubitril valsartan alone, and the observation group was treated with sacubitril valsartan+rhBNP. The clinical efficacy, readmission rate within 2 months, serum indexes [N-terminal pro-brain natriuretic peptide (NT-proBNP), soluble human matrix lysin 2 (sST2), high-sensitivity C-reactive protein (hsCRP), troponinⅠ (TnⅠ)], renal function indexes [urea nitrogen, serum creatinine, glomerular filtration rate (eGFR), urine volume] and echocardiography indexes [left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular ejection fraction (LVEF)] were compared between the two groups. Results The total effective rate of the observation group (94.29%) was higher than that of the control group (68.57%), and the readmission rate within 2 months (8.57%) was lower than that of the control group (31.43%) (P<0.05). After treatment, NT-proBNP, sST2, hs-CRP and TnⅠ in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of urea nitrogen and serum creatinine in the observation group were lower than those in the control group, eGFR and urine volume were higher than those in the control group, LVEDD and LVESD were lower than those in the control group, and LVEF was higher than that in the control group (P<0.05). Conclusion The combination of rhBNP and sacubitril valsartan can improve the cardiac function of patients with refractory heart failure, reduce the levels of serum inflammatory and myocardial injury markers, and protect renal function, which is worthy of clinical application.
