摘要:
目的 通过对某三级综合医院输血病历质量进行检查，分析输血病历质量的影响因素，不断提高输血病历的质量。方法 采用PDCA四步法，以2015年第2季度~第4季度实施PDCA循环前的252份输血病历作为对照组，2016年第2季度~第4季度实施PDCA循环后的228份输血病历作为实验组，对比两组改进前后的病历质量。结果 实验组在输血病程记录缺陷（27.78%），发血报告单缺陷（13.89%），输血知情同意书缺陷（13.10%），无指征输血（2.78%）等方面的缺陷率均大幅度下降，低于对照组，差异具有统计学意义（P＜0.05）。结论 PDCA四步法应用于输血病历质量持续改进，能有效降低各项输血病历指标缺陷率，提高病历质量。
Abstract:
Abstract：Objective To improve the quality of blood transfusion medical records by examining the quality of transfusion records in a tertiary general hospital and analyzing the influencing factors of the quality of blood transfusion medical records.Methods Using the PDCA four-step method,252 transfusion records before the PDCA cycle were implemented as the control group from the second quarter to the fourth quarter of 2015.228 transfusion records after the PDCA cycle were implemented in the second quarter to the fourth quarter of 2016 as the experimental group.Compare the quality of medical records before and after improvement between the two groups.Results In the experimental group,the defect rate of the blood transfusion disease record(27.78%),the blood report ring defect(13.89%),the blood transfusion informed consent defect(13.10%),and the non-indication blood transfusion(2.78%)decreased significantly,lower than control group,the difference was statistically significant(P<0.05).Conclusion The four-step PDCA method is applied to the continuous improvement of the quality of blood transfusion medical records,which can effectively reduce the defect rate of various blood transfusion medical records and improve the quality of medical records.
