摘要:
目的 采用Meta分析探讨服用阿司匹林是否改善乳腺癌患者的预后。方法 检索PubMed、EMBASE、CNKI等中外数据库中关于服用阿司匹林与乳腺癌预后关系的文献,时间截止至2018年3月,提取全因死亡及乳腺癌特异死亡的相关结果进行效应量的合并,对纳入的数据进行敏感性分析、发表偏倚分析等。所有统计分析均采用Stata12.0软件完成。结果 共纳入11篇文献,其中包括9篇队列研究及2篇巢式病例对照研究,共计乳腺癌患者36862例。Meta分析结果显示,诊断前服用阿司匹林的乳腺癌患者与未服用的患者相比,无论是总死亡率(HR=1.06,95%CI=0.78~1.43)还是肿瘤特异死亡率(HR=1.06,95%CI=0.66~1.69)均无显著改善。诊断后服用阿司匹林并不能降低乳腺癌患者的总死亡率(HR=0.84,95%CI=0.64~1.11),但可降低肿瘤特异死亡率(HR=0.73,95%CI=0.55~0.98)。对纳入文献进行敏感性分析,结果显示,诊断后服用阿司匹林并不能降低肿瘤特异死亡率(HR=0.95,95%CI=0.85~1.06)。结论 服用阿司匹林对改善乳腺癌患者的死亡率无显著作用。
Abstract:
Objective To investigate whether taking aspirin improves the prognosis of breast cancer patients by meta-analysis. Methods The literatures on the relationship between aspirin and breast cancer prognosis in PubMed,EMBASE,CNKI and other Chinese and foreign databases were searched.The results of the combination of all-cause death and breast cancer-specific death were combined until March 2018.The data is subjected to sensitivity analysis,publication bias analysis,and the like.All statistical analyses were performed using Stata 12.0 software.Results A total of 11 articles were included,including 9 cohort studies and 2 nested case-control studies,with a total of 36,862 breast cancer patients.Meta analysis showed that there was no significant improvement in total mortality(HR=1.06,95%CI=0.78~1.43)or tumor specific mortality(HR=1.06,95%CI=0.66~1.69)in patients with breast cancer who took aspirin before diagnosis compared with those who did not.Taking aspirin after diagnosis did not reduce the overall mortality of breast cancer patients(HR=0.84,95%CI=0.64~1.11),but reduced tumor-specific mortality(HR=0.73,95%CI=0.55~0.98).Sensitivity analysis of the included literature showed that taking aspirin after diagnosis did not reduce tumor-specific mortality(HR=0.95,95%CI=0.85~1.06). Conclusion Taking aspirin has no significant effect on improving mortality in breast cancer patients.
