摘要:
目的 了解2008年~2017年在枝江市精神卫生中心住院的首发精神障碍患者病种构成和变化情况，为今后精神卫生工作方向提供参考。方法 选择2008年1月1日~2017年12月31日在我院住院的首发精神障碍患者病历资料进行回顾性分析，了解各年各病种的构成和变化，分析其结果。结果 共有4888例首发精神障碍患者纳入研究，男性2133例（43.64%），女性2755（56.36%），平均年龄为（43.96±16.13）岁，平均住院时间（42.50±36.04）d。精神疾病构成比前三位为：精神分裂症或其他精神病性障碍、神经症、心境障碍。十年间各个病种构成变化方面，精神分裂症或其他精神病性障碍总体呈下降趋势，变化具有统计学意义（P＜0.05），神经症构成比总体呈上升趋势，变化具有统计学意义（P＜0.05），其他几类疾病的构成比在十年间无明显变化。结论 神经症正逐渐取代精神分裂症成为最多占比病种，应当进一步提高心理卫生服务能力，应对在接下来几年里可能出现的住院病种谱的变化。
Abstract:
Objective To understand the composition and changes of the first-episode mentally ill patients hospitalized in Zhijiang Mental Health Center from 2008 to 2017,and provide reference for the future mental health work.Methods The medical records of the first-episode mentally ill patients hospitalized in our hospital from January 1,2008 to December 31,2017 were retrospectively analyzed to understand the composition and changes of each disease in each year,and the results were analyzed.Results A total of 4888 patients with first-episode mental disorders were included in the study,2133(43.64%)males,2755(56.36%)females,mean age (43.96±16.13)years,and mean hospital stay(42.50±36.04)d.The mental illness constitutes the top three: schizophrenia or other psychotic disorders,neurosis,mood disorders.In the ten years of changes in the composition of various diseases,schizophrenia or other psychiatric disorders generally showed a downward trend,the change was statistically significant(P<0.05),and the neurological composition was generally higher than the overall trend,and the change was statistically significant(P<0.05),the composition of other types of diseases did not change significantly during the decade.Conclusion Neurosis is gradually replacing schizophrenia as the most common disease.It should further improve the mental health service ability and cope with the changes in the spectrum of hospitalization diseases that may occur in the next few years.
