摘要:
目的 分析我院694例角膜移植患者手术原因及变化特点。方法 选择2015年6月~2018年5月在我院行角膜移植患者694例，分析患者手术原因及变化趋势。结果 近3年来我院行角膜移植原因依次为角膜瘢痕（40.34%）、圆锥角膜（15.41%）、感染性角膜炎（11.81%）、角膜内皮失代偿（8.93%）、再次手术（8.21%）和角膜营养不良（3.89%）；感染性角膜炎行角膜移植比例增加（P＜0.05），角膜内皮失代偿而手术比例逐年减少（P＜0.05），角膜瘢痕、再次手术和角膜营养不良手术比例无明显变化（P均＞0.05）；角膜瘢痕以病毒性角膜白斑为主（36.78%），感染性角膜炎以真菌为主（48.78%）。结论 角膜瘢痕仍为角膜移植首要原因，因感染性角膜炎行角膜移植的变化值得关注，上述两者均为可预防因素，目前仍需大力宣传眼卫生以及职业防护。
Abstract:
Abstract：Objective To analyze the causes and changes of 694 corneal transplant patients in our hospital.Methods A total of 694 patients with corneal transplantation in our hospital from June 2015 to May 2018 were selected to analyze the causes and trends of the patients.Results In the past 3 years, the causes of corneal transplantation in our hospital were corneal scar (40.34%), keratoconus (15.41%), infectious keratitis (11.81%), corneal endothelium decompensation (8.93%), and reoperation (8.21%), and corneal dystrophy (3.89%); increased proportion of corneal transplantation for infective keratitis (P<0.05), corneal endothelium decompensation and the proportion of surgery decreased year by year (P<0.05), corneal scar, reoperation and corneal dystrophy there was no significant change in the proportion of surgery (Pmean>0.05); corneal scar was mainly viral leukoplakia (36.78%), and infectious keratitis was mainly fungi (48.78%).Conclusion Corneal scar is still the primary cause of corneal transplantation. The changes of corneal transplantation due to infectious keratitis are worthy of attention. Both of them are preventable factors. At present, eye health and occupational protection are still needed.
