摘要:
目的 分析血清免疫固定电泳、血清蛋白电泳、免疫球蛋白定量对多发性骨髓瘤(MM)的临床诊断。方法 选取2017年5月~2018年5月在我院门诊或住院部收治的多发性骨髓瘤患者100例作为案例。所有患者均接受血清免疫固定电泳、血清蛋白电泳、免疫球蛋白定量检查。分析MM患者血清总蛋白、免疫球蛋白、血清蛋白电泳以及免疫固定电泳的异常发生率以及IgG、IgA、IgM的K、入型检查结果。结果 100例多发性骨髓瘤患者中72例患者异常,其中42例患者属于血清总蛋白量提升,30例患者属于球蛋白含量提升、白蛋白含量下降,总蛋白含量正常。血清免疫球蛋白定量检测中36例IgG含量提升,19例患者IgA含量提升,4例单纯轻链性免疫球蛋白IgG、IgA以及IgM正常或稍微下降。100例患者当中63例患者的血清蛋白电泳图谱中存在M蛋白。结论 血清免疫固定电泳、血清蛋白电泳、免疫球蛋白定量对多发性骨髓瘤的临床诊断价值较高,多发性骨髓瘤可以先用血清蛋白电泳与免疫球蛋白定量实现初步筛查,之后采用免疫固定电泳实现临床诊断与鉴别,整体诊断效果理想。
Abstract:
Abstract:Objective To analyze the clinical diagnosis of multiple myeloma (MM) by serum immunofixation electrophoresis, serum protein electrophoresis and immunoglobulin quantification. Methods A total of 100 patients with multiple myeloma admitted to our hospital from May 2017 to May 2018 were selected as examples. All patients received serum immunofixation electrophoresis, serum protein electrophoresis, immunoglobulin quantitative examination. The abnormal incidence rate of serum total protein, immunoglobulin, serum protein electrophoresis and immunofixation electrophoresis in MM patients, as well as K of IgG, IgA, IgM, and intestation results were analyzed. Results Of the 100 patients with multiple myeloma, 72 were abnormal. Among them, 42 patients had an increase in total serum protein, and 30 patients had elevated globulin content, decreased albumin content, and normal total protein content. In the serum immunoglobulin quantitative assay, 36 cases of IgG increased, 19 cases of IgA increased, 4 cases of light chain immunoglobulin IgG, IgA and IgM normal or slightly decreased. M protein was present in the serum protein electropherogram of 63 of the 100 patients.Conclusion Serum immunofixation electrophoresis, serum protein electrophoresis and immunoglobulin quantification have high clinical value in the diagnosis of multiple myeloma. Multiple myeloma can be initially screened by serum protein electrophoresis and immunoglobulin quantification, followed by immunofixation. Electrophoresis realizes clinical diagnosis and identification, and the overall diagnosis results are ideal.
