目的 了解通用航空事故及事故征候对飞行员以及事故外其他飞行员造成心理影响甚至创伤后应激障碍(PTSD)。方法选取2018年8月~12月来我院体检的西北地区通用航空飞行员106人,根据所在单位是否发生飞行事故分为事故单位组(n=49)和非事故单位组(n=57),调查PTSD状况,对比事件影响量表修订版(IES-R)[分为唤醒(H)、逃避(A)、侵扰(I)三个因子]、抑郁自评量表(SDS)和焦虑自评量表(SAS)三种量表得分,并用认知心理干预改善飞行员正性情感,改善心理应激带来的不良影响。结果 事故单位组IES-R、IES-R(I)、IES-R(H)、SAS、SDS量表前测得分均高于非事故单位组,差异有统计学有意义(P<0.05);两组IES-R(A)量表前测得分比较,差异无统计学意义(P>0.05)。所有飞行员IES-R平均得分为(33.26±11.27)分,对筛选出符合PTSD标准的43名飞行员进行认知心理干预。经过认知心理干预后,IES-R、IES-R(I)、IES-R(H)、SAS、SDS量表得分均低于干预前,差异有统计学有意义(P<0.05);干预前后IES-R(A)量表得分比较,差异无统计学意义(P>0.05)。结论 认知心理干预可改善通用航空飞行员PTSD,让其认识自己情绪、思维和行为的消极因素,增强其信心,从而达到改善病情的目的。
Abstract:
Abstract:Objective To understand the general effects of general aviation accidents and incidents on pilots and other pilots outside the accident, and even post-traumatic stress disorder (PTSD). Methods A total of 106 general aviation pilots from the northwestern region who came to our hospital from August to December 2018 were selected. According to whether the unit was involved in flight accidents, it was divided into accident unit group (n=49) and non-accident unit group (n=57). Investigate PTSD status, compare event impact scale revision (IES-R) [divide into three factors: arousal (H), escape (A), intrusion (I)], self-rating depression scale (SDS) and self-evaluation of anxiety The scales (SAS) scores on three scales, and use cognitive psychological intervention to improve the positive emotions of the pilots and improve the adverse effects of psychological stress.Results The scores of the IES-R, IES-R(I), IES-R(H), SAS and SDS scales in the accident unit group were higher than those in the non-accident unit group, the difference was statistically significant (P<0.05). There was no significant difference in the pre-test scores between the two groups on the IES-R(A) scale (P>0.05). The average score of the IES-R for all pilots was (33.26±11.27) points, and cognitive psychology intervention was performed on 43 pilots who were selected to meet the PTSD criteria. After cognitive intervention, the scores of IES-R, IES-R(I), IES-R(H), SAS, and SDS were lower than those before intervention,the difference was statistically significant (P<0.05). Before and after the IES-R (A) scale scores, the difference was not statistically significant (P>0.05).Conclusion Cognitive psychological intervention can improve the general aviation pilot PTSD, let them know the negative factors of their emotions, thinking and behavior, enhance their confidence, and thus achieve the purpose of improving the condition.
