摘要:
目的 研究尼莫地平不同时间用药对脑出血患者局部脑血流、脑水肿及临床变化的影响。方法 选取2017年4月~2018年4月在我院诊治的96例脑出血患者,将其随机分为4组,A组为发病24 h内给予尼莫地平治疗(对照组),B组发病24~48 h给药、C组发病48~72 h给药、D组发病72 h后给药,比较各组治疗后局部水肿体积、神经功能缺损评分、生活能力评分等改善情况。结果 与对照组对比,随着给药时间推移,病灶水肿体积、神经功能缺损评分、缺血体积等指标逐渐增加,而生活能力评分逐渐降低,差异有统计学意义(P<0.05)。给药时间越早,尼莫地平对脑血流、脑水肿的改善效果越好,其差异显著(P<0.05)。结论 对于脑出血患者采用尼莫地平治疗,可以改善局部血流、脑水肿状况,减轻神经功能缺损,且24 h内用药治疗可以获得更加的疗效和预后效果。
Abstract:
Objective To investigate the effects of nimodipine at different times on regional cerebral blood flow, brain edema and clinical changes in patients with cerebral hemorrhage. Methods 96 patients with cerebral hemorrhage diagnosed in our hospital from April 2017 to April 2018 were randomly divided into 4 groups. Group A (control group)was treated with nimodipine within 24 h after onset, and group B was affected for 24 to 48 h. The drug was administered in group C for 48-72 h, and group D administered 72 h after onse. The improvement of local edema volume, neurological deficit score, and life ability score after treatment were compared. Results Compared with the control group, the lesion edema volume, neurological deficit score, ischemic volume and other indicators gradually increased with the administration time, while the life ability score gradually decreased, the difference was statistically significant (P<0.05). The earlier the administration time, the better the effect of nimodipine on cerebral blood flow and cerebral edema, and the difference was significant (P<0.05). Conclusion Nimodipine treatment for patients with cerebral hemorrhage can improve local blood flow, cerebral edema and reduce neurological deficits, and more effective and prognostic effects can be obtained within 24 h.
