摘要:
目的 分析健康体检者胰岛素抵抗(IR)与脂代谢异常之间的相关性,并探讨应用甘油三酯/高密度脂蛋白胆固醇(TG/HDL-C)预测IR的最佳分界值。方法 选取2014年5月~7月于河北省人民医院体检中心体检者785名纳入本研究,按照不同性别将研究对象分别以胰岛素抵抗指数(HOMA-IR)中位数2.02为界限,分为胰岛素抵抗(IR)组和胰岛素敏感(IS)组,比较两组体格检查指标及生化指标,并分析IR与血脂异常的相关性。应用ROC曲线探讨TG/HDL-C预测IR的最佳分界值。 结果 两组男性和女性空腹血糖(FBG)、总胆固醇(TC)、TG、HDL-C、低密度脂蛋白胆固醇(LDL-C)水平及TG/HDL-C比较,差异有统计学意义(P<0.05)。Spearman相关分析显示,HOMA-IR与FBG、TC、TG、LDL-C、TG/HDL-C呈正相关(P<0.05),与HDL-C呈负相关(P<0.05)。Logistic回归分析结果显示年龄、体重指数(BMI)、腰围(WC)、FBG、TG及HDL-C是IR的危险因素。ROC曲线结果提示预测男性健康体检者IR发生风险的TG/HDL-C为1.24(AUC:0.748,95%CI:0.704~0.792),敏感性为64.80%,特异性为75.90%;预测女性健康体检者IR发生风险的TG/HDL-C为1.36(AUC:0.728,95%CI:0.670~0.785),敏感性为58.00%,特异性为79.70%。结论 IR与血脂异常具有相关性,尤其和TG、HDL-C密切相关。TG/HDL-C可以作为预测健康体检者IR的一个指标。
Abstract:
Objective To analyze the correlation between insulin resistance (IR) and abnormal lipid metabolism in healthy people, and to explore the best cut-off value for IR using triglyceride / high density lipoprotein cholesterol (TG / HDL-C). Methods From May to July 2014, 785 people who had undergone physical examinations at the Hebei Provincial People’s Hospital Physical Examination Center were included in this study. The subjects were divided into insulin resistance with the median insulin resistance index (HOMA-IR) of 2.02 according to different genders. (IR) group and insulin sensitivity (IS) group, physical examination indicators and biochemical indicators of the two groups were compared, and the correlation between IR and dyslipidemia was analyzed. Using ROC curve to explore the best cut-off value of IR predicted by TG/HDL-C.Results There were statistically significant differences in fasting blood glucose (FBG), total cholesterol (TC), TG, HDL-C, low-density lipoprotein cholesterol (LDL-C) and TG / HDL-C between the two groups of men and women,the difference was statistically significant(P<0.05). Spearman correlation analysis showed that HOMA-IR was positively correlated with FBG, TC, TG, LDL-C, and TG/HDL-C(P<0.05) and negatively correlated with HDL-C(P<0.05).Logistic regression analysis showed that age, body mass index (BMI), waist circumference(WC), FBG, TG, and HDL-C were risk factors for IR. The results of the ROC curve suggest that the TG/HDL-C for predicting the risk of IR in male health check-ups is 1.24 (AUC: 0.748, 95%CI: 0.704~0.792), the sensitivity is 64.80%, and the specificity is 75.90%; predicting female health check The TG/HDL-C risk of IR was 1.36(AUC:0.728,95%CI:0.670-0.785), the sensitivity was 58.00%, and the specificity was 79.70%. Conclusion IR is associated with dyslipidemia, especially with TG and HDL-C. TG / HDL-C can be used as an indicator to predict the IR of health examiners.
