摘要:
目的 探讨CT动静脉造影在上矢状窦旁脑膜瘤术前评估中的应用价值。方法 回顾性分析2016年1月~2019年12月我院确诊的36例上矢状窦旁脑膜瘤CT动静脉造影资料,分析引流静脉显示情况、肿瘤血管形态、侧枝循环形成情况以及肿瘤与上矢状窦之间的关系等影像学特征。结果 共36例颅内脑外上矢状窦旁脑膜瘤,位于上矢状窦前1/3范围13例,中后1/3范围23例;位于上矢状窦左侧9例,位于右侧16例,骑跨上矢状窦11例;肿瘤直径1.32~6.88 cm,平均肿瘤直径(3.53±0.59)cm;通过三维后处理可清晰显示瘤体血管情况以及供血动脉、引流静脉以及侧枝循环情况;肿瘤与上矢状窦存在间隙6例,贴近毗邻13例,单纯压迫伴矢状窦部分狭窄12例,包绕侵犯5例。结论 CT动静脉造影可以清晰显示上矢状窦旁脑膜瘤引流静脉显示情况、肿瘤血管形态、侧枝循环形成情况以及肿瘤与上矢状窦之间的关系等心态学特征,可有效指导手术入路的选择。
Abstract:
Objective To explore the value of CT arteriovenous angiography in the preoperative evaluation of superior sagittal sinus meningioma. Methods Retrospective analysis of CT arteriovenous angiography data of 36 cases of superior sagittal parasinus meningioma diagnosed in our hospital from January 2016 to December 2019. The drainage vein display, tumor vessel morphology, collateral circulation formation, and tumor The relationship between sagittal sinus and other imaging features.Results A total of 36 cases of intracranial and extracranial superior sagittal sinus meningiomas were located in 13 cases in the anterior 1/3 of the superior sagittal sinus and 23 cases in the middle and posterior 1/3 of the superior sagittal sinus; 9 cases were located on the left side of the superior sagittal sinus, 16 cases were on the right side, and 11 cases were riding on the superior sagittal sinus. The tumor diameter was 1.32 to 6.88 cm, and the average tumor diameter was (3.53±0.59) cm. The tumor was clearly displayed through three-dimensional post-processing. Body blood vessels and blood supply arteries, draining veins, and collateral circulation; 6 cases had tumors and superior sagittal sinus, 13 cases were adjacent,compression with sagittal sinus stenosis in 12 cases was simple compression, and 5 cases were encircled.Conclusion CT arteriovenous angiography can clearly show the situation of drainage vein display of superior sagittal paranasal meningioma, tumor vascular morphology, lateral branch circulation formation and the relationship between tumor and superior sagittal sinus, effectively guide the choice of surgical approach.
