摘要:
目的 系统评价皮肤镜对基底细胞癌的诊断价值。方法 计算机检索皮肤镜诊断基底细胞癌的的相关文献,检索数据库为PubMed、SinoMed、Web of Science、Science of Direct、中国知网、万方数据库和维普中文科技期刊数据库,检索时间为建库至2020年4月,按照QUADAS标准对纳入文献进行质量评估,使用STATA14.0软件进行Meta分析及Meta回归分析,对其敏感度、特异度进行汇总,行敏感性分析及异质性检验,绘制综合受试者工作特征曲线(SROC)。结果 最终纳入20篇文献,包括3108例样本。采用随机效应模型进行Meta分析,结果显示皮肤镜诊断基底细胞癌的合并敏感度为0.92(95%CI:0.88~0.94),特异度为0.90(95%CI:0.85~0.94),阳性似然比为8.17(95%CI:5.14~12.98),阴性似然比为0.10(95%CI:0.06~0.14),诊断比值比为103.4(95%CI:49.53~215.78);SROC曲线下面积为0.97。结论 皮肤镜对基底细胞癌有较好的敏感度和特异度,诊断价值较高,但临床工作中仍需结合其他相关检查及具体临床表现进行诊断。
Abstract:
Objective To systematically evaluate the diagnostic value of dermoscopy for basal cell carcinoma.Methods A computer search of related literature on the diagnosis of basal cell carcinoma by dermoscopy. The search databases were PubMed, SinoMed, Web of Science, Science of Direct, CNKI, Wanfang database and Weipu Chinese science and technology journal database.The retrieval time is from the establishment of the database to April 2020. The quality of the included documents is evaluated according to the QUADAS standard. The STATA14.0 software is used for Meta analysis and Meta regression analysis. The sensitivity and specificity are summarized, and sensitivity analysis and difference are performed. Qualitative inspection, drawing a comprehensive receiver operating characteristic curve (SROC).Results 20 articles were finally included, including 3108 samples. Meta-analysis using random effects model showed that the combined sensitivity of dermoscopy for diagnosis of basal cell carcinoma was 0.92 (95%CI: 0.88~0.94), specificity was 0.90 (95%CI: 0.85~0.94), and the positive likelihood ratio was 8.17 (95%CI: 5.14~12.98), the negative likelihood ratio was 0.10 (95%CI: 0.06~0.14), the diagnostic odds ratio was 103.4 (95%CI: 49.53~215.78); the area under the SROC curve was 0.97.Conclusion Dermoscopy has good sensitivity and specificity for basal cell carcinoma, and has a high diagnostic value. However, it is still necessary to combine other related examinations and specific clinical manifestations for diagnosis in clinical work.
