摘要:
目的 评价高频超声联合低频超声诊断急性阑尾炎的效果。方法 收集2018年5月~2019年6月我院病理确诊的80例急性阑尾炎患者影像学资料，其中单纯性阑尾炎12例，坏疽性阑尾炎9例，化脓性阑尾炎48例，阑尾周围脓肿11例，比较高频超声、低频超声及高频联合低频超声诊断的准确率、敏感度、特异性以及超声征象显示情况。结果 联合超声诊断单纯性阑尾炎、坏疽性阑尾炎、化脓性阑尾炎、阑尾周围脓肿的准确率均高于高频超声和低频超声，差异有统计学意义（P＜0.05）。阑尾炎超声表现分为直接征象：阑尾显著增粗且直径大于7 mm、壁增厚且壁厚大于2 mm、腔内有强回声粪石影；间接征象：阑尾肿大探头加压时出现压痛和反跳痛、阑尾周围出现积液、阑尾周围有高回声网膜包绕、回盲部和周围肠壁较厚、右下腹肠管蠕动速度减弱甚至逐步消失，肠腔内有积液。结论 高频联合低频超声诊断可提高急性阑尾炎诊断准确率，且可准确分型，清晰显示阑尾情况，可为临床及时有效治疗提供可靠的参考依据。
Abstract:
Objective To evaluate the effect of high frequency ultrasound combined with low frequency ultrasound in the diagnosis of acute appendicitis.Methods The imaging data of 80 patients with acute appendicitis diagnosed pathologically in our hospital from May 2018 to June 2019 were collected, including 12 cases of simple appendicitis, 9 cases of gangrenous appendicitis, 48 cases of suppurative appendicitis, and 11 cases of periappendic abscess. Compare the accuracy, sensitivity, specificity and the display of ultrasound signs of high-frequency ultrasound, low-frequency ultrasound and high-frequency combined with low-frequency ultrasound.Results The accuracy of combined ultrasound in the diagnosis of simple appendicitis, gangrenous appendicitis, suppurative appendicitis, and peri-appendicular abscess was higher than that of high-frequency ultrasound and low-frequency ultrasound,the difference was statistically significant (P<0.05). Ultrasound manifestations of appendicitis are divided into direct signed: the appendix was significantly enlarged and the diameter was greater than 7 mm, the wall was thickened and the wall thickness was greater than 2 mm, and there was a strong echo of fecal stone shadow in the cavity; indirect signed: tenderness and tenderness when the probe was compressed rebound pain, fluid effusion around the appendix, hyperechoic omentum around the appendix, thicker ileocecal area and surrounding intestinal wall, lower right lower abdominal bowel peristalsis or even disappear gradually, and fluid accumulation in the intestinal cavity.Conclusion High-frequency combined with low-frequency ultrasound diagnosis can improve the diagnostic accuracy of acute appendicitis, and can accurately type and clearly display the appendix, which can provide a reliable reference for clinical timely and effective treatment.
