摘要:
目的 研究添加β-内酰胺酶血培养瓶对含抗菌素模拟儿童菌血症标本检测能力。方法 将培养瓶分为BD-S(BD BACTEC FX血培养瓶)、BD-F(BD BACTEC Peds Plus血培养瓶)、BD-SE(BD-S添加β-内酰胺酶)及BT-S(BacT/ALERT SA血培养瓶)、BT-C(BACT/ALERT PF血培养瓶)、BT-SE(BT-S添加β-内酰胺酶),选择儿科常见病原菌和常用抗菌素进行配对分组(金黄色葡萄球菌-青霉素钠组和大肠埃希菌-头孢呋辛钠组),将菌液、抗菌素(血药峰和中点浓度)、全血注入上述培养瓶进行培养,比较不同病原菌-抗生素分组的检出阳性率和阳性报菌时间(TTP)。结果 金黄色葡萄球菌-青霉素钠组和大肠埃希菌-头孢呋辛钠组的检出阳性率:二者血药峰浓度时,BD-F、BD-SE均优于BD-S,差异有统计学意义(P<0.05);头孢呋辛钠峰浓度时,BT-C、BT-SE优于BT-S,差异有统计学意义(P<0.05);二者峰和中点浓度时,BD-F与BD-SE、BT-C与BT-SE比较,差异无统计学意义(P>0.05)。TTP:二者峰和中点浓度时,BD-F、BD-SE均优于BD-S,差异有统计学意义(P<0.05);BT-C、BT-SE均优于BT-S,差异有统计学意义(P<0.05);青霉素钠中点浓度时,BD-SE优于BD-F,差异有统计学意义(P<0.05);青霉素钠峰浓度时,BD-F优于BD-SE,差异有统计学意义(P<0.05);青霉素钠峰和中点浓度时,BT-C优于BT-SE,差异有统计学意义(P<0.05);头孢呋辛钠峰和中点浓度时,BT-C与BT-SE比较,差异无统计学意义(P>0.05)。结论 添加了β-内酰胺酶的培养瓶对于含抗菌素模拟儿童菌血症标本检验效能优于普通培养瓶,并且与含有树脂、活性炭的培养瓶检验效能接近。
Abstract:
Objective To study the ability of adding β-lactamase blood culture flasks to detect bacteremia samples containing antibiotics in children.Methods The culture bottles were divided into BD-S (BD BACTEC FX blood culture bottles), BD-F (BD BACTEC Peds Plus blood culture bottles), BD-SE (BD-S with β-lactamase) and BT-S (BacT/ALERT SA blood culture bottle), BT-C (BACT/ALERT PF blood culture bottle), BT-SE (BT-S with β-lactamase),selecting common pediatric pathogens and commonly used antibiotics for pairing (Staphylococcus aureus-penicillin sodium group and Escherichia coli-cefuroxime sodium group),inject bacteria liquid, antibiotics (blood drug peak and midpoint concentration), and whole blood into the above-mentioned culture bottle for culture,compare the positive detection rate and the positive reporting time (TTP) of different pathogen-antibiotic groups.Results The positive rate of detection of Staphylococcus aureus-penicillin sodium group and Escherichia coli-cefuroxime sodium group: BD-F and BD-SE are better than BD-S at the peak blood concentration of both,the difference was statistically significant (P<0.05);At the peak concentration of cefuroxime sodium, BT-C and BT-SE were better than BT-S,the difference was statistically significant (P<0.05);At the peak and midpoint concentrations of the two, there was no statistically significant difference between BD-F and BD-SE, and BT-C and BT-SE (P>0.05).TTP: BD-F and BD-SE were better than BD-S at both peak and midpoint concentrations,the difference was statistically significant (P<0.05);BT-C and BT-SE were better than BT-S, the difference was statistically significant (P<0.05);At the midpoint concentration of penicillin sodium, BD-SE was better than BD-F,the difference was statistically significant (P<0.05);At the peak concentration of penicillin sodium, BD-F was better than BD-SE,the difference was statistically significant (P<0.05);At the peak and midpoint concentrations of penicillin sodium, BT-C was better than BT-SE,the difference was statistically significant (P<0.05);There was no statistically significant difference between BT-C and BT-SE at the peak and midpoint concentrations of cefuroxime sodium (P>0.05).Conclusion The test efficiency of culture flasks with β-lactamase added is better than that of ordinary culture flasks for bacteremia samples containing antibiotics, and is similar to that of culture flasks containing resin and activated carbon.
