摘要:
目的 研究PLR与不稳定心绞痛患者冠脉病变严重程度的相关性。方法 回顾性分析2018年1月~2019年12月于石河子大学第一附属医院明确诊断为不稳定心绞痛(UA)230例,根据SYNTAX评分分为冠状动脉轻度病变组(SYNTAX评分<23分)137例和冠状动脉中重度病变组(SYNTAX评分≥23分)93例,比较两组一般资料及实验室检查结果,通过二元Logistic回归分析UA患者冠状动脉严重程度的危险因素,采用Spearman相关性分析LDL-C/HDL-C、PLR与冠状动脉SYNTAX评分的关系,绘制受试者工作特征(ROC)曲线分析不同指标对UA患者冠状动脉严重程度的诊断价值。结果 ①两组年龄、性别、高血压病史、糖尿病病史、吸烟史比较,差异均无统计学意义(P>0.05);两组ApoA1、ApoB、TC、HDL、LDL、ApoB/ApoA1、TC/HDL-C、LDL-C/HDL-C、PLR比较,差异均有统计学意义(P<0.05);②单因素及多因素Logistic回归分析显示,LDL-C/HDL-C、PLR是UA患者冠状动脉病变严重程度的独立危险因素;③Spearman相关性分析显示,LDL-C/HDL-C、PLR与UA患者SYNTAX评分呈正相关(r=0.397、0.373,P<0.05);④LDL-C/HDL-C、PLR诊断冠状动脉病变严重程度的ROC曲线下面积分别为0.733、0.719,两项指标联合诊断冠状动脉病变严重程度的ROC曲线下面积为0.785。结论 LDL-C/HDL-C、PLR与UA患者冠状动脉病变严重程度密切相关,可作为判断UA患者冠状动脉病变严重程度的独立危险因素,两者联合使用对冠状动脉病变严重程度的预测准确度较高。
Abstract:
Objective To study the correlation between PLR and the severity of coronary artery disease in patients with unstable angina pectoris.Methods A retrospective analysis of 230 cases of unstable angina (UA) diagnosed in the First Affiliated Hospital of Shihezi University from January 2018 to December 2019.According to the SYNTAX score, they were divided into a mild coronary artery disease group (SYNTAX score<23 points) 137 cases and a moderate to severe coronary artery disease group (SYNTAX score≥23 points) 93 cases.Compare the general information and laboratory test results between the two groups,binary Logistic regression was used to analyze the risk factors of coronary artery severity in UA patients, and Spearman correlation was used to analyze the relationship between LDL-C/HDL-C, PLR and coronary SYNTAX score.Draw receiver operating characteristic (ROC) curves to analyze the diagnostic value of different indicators for coronary artery severity in UA patients.Results ① There was no significant difference in age, gender, history of hypertension, history of diabetes, and smoking history between the two groups (P>0.05);Comparing the two groups of ApoA1, ApoB, TC, HDL, LDL, ApoB/ApoA1, TC/HDL-C, LDL-C/HDL-C, PLR, the differences were statistically significant (P<0.05);②Single factor and multivariate Logistic regression analysis showed that LDL-C/HDL-C and PLR are independent risk factors for the severity of coronary artery disease in UA patients;③Spearman correlation analysis showed that LDL-C/HDL-C, PLR and UA patients’ SYNTAX score were positively correlated (r=0.397, 0.373, P<0.05);④The area under the ROC curve for LDL-C/HDL-C and PLR to diagnose the severity of coronary artery disease was 0.733 and 0.719, respectively, and the area under the ROC curve for the combined diagnosis of the severity of coronary artery disease by the two indicators was 0.785.Conclusion LDL-C/HDL-C and PLR are closely related to the severity of coronary artery disease in UA patients, and can be used as independent risk factors for judging the severity of coronary artery disease in UA patients.The combination of the two has a higher accuracy in predicting the severity of coronary artery disease.
