摘要:
目的 观察骨填充网袋椎体成形术(BFMCs)治疗骨质疏松性椎体压缩性骨折(OVCF)的临床效果。方法 选取2018年1月~2020年1月我院收治的OVCF患者65例作为研究对象,按照手术方法分为对照组32例和观察组33例。对照组采用椎体后凸成形术(PKP)治疗,观察组采用BFMCs治疗,比较两组疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)、伤椎前缘高度、椎体中部高度、后凸Cobb角、血清胰岛素样生长因子(IGF)、血清瘦素(leptin)、脂联素(APN)及并发症发生情况。结果 观察组VAS评分、ODI低于对照组,伤椎前缘高度、椎体中部高度高于对照组,后凸Cobb角小于对照组,差异均有统计学意义(P<0.05);术后1个月,两组血清IGF、APN、leptin较术前改善,且观察组优于对照组,差异均有统计学意义(P<0.05);观察组骨水泥渗漏发生率(3.03%)低于对照组(21.88%),差异有统计学意义(P<0.05)。结论 骨填充网袋椎体成形术治疗骨质疏松性椎体压缩性骨折效果确切,能够恢复伤椎高度,减轻疼痛,改善脊柱功能及血清IGF、APN、leptin水平,减少骨水泥渗漏,安全性良好。
Abstract:
Objective To observe the clinical effect of bone-filled mesh pocket vertebroplasty (BFMCs) in the treatment of osteoporotic vertebral compression fractures (OVCF).Methods A total of 65 OVCF patients admitted to our hospital from January 2018 to January 2020 were selected as the research objects. According to the surgical method, they were divided into 32 cases in the control group and 33 cases in the observation group.The control group was treated with kyphoplasty (PKP), and the observation group was treated with BFMCs.Comparing the two groups of pain visual analog scale (VAS), Oswestry dysfunction index (ODI), height of the anterior edge of the injured vertebral body, mid-vertebral body height, kyphotic Cobb angle, serum insulin-like growth factor (IGF), serum leptin (leptin),Adiponectin (APN) and complications.Results The VAS score and ODI of the observation group were lower than those of the control group. The height of the anterior edge of the injured vertebrae and the height of the middle of the vertebral body were higher than those of the control group. The Cobb angle of kyphosis was smaller than that of the control group,the differences were statistically significant (P<0.05);One month after operation, the serum IGF, APN and leptin of the two groups were improved compared with preoperatively, and the observation group was better than the control group, the difference was statistically significant (P<0.05);The incidence of bone cement leakage in the observation group (3.03%) was lower than that in the control group (21.88%),the difference was statistically significant (P<0.05).Conclusion Bone filling mesh bag vertebroplasty is effective in treating osteoporotic vertebral compression fractures.It can restore the height of the injured vertebrae, relieve pain, improve spinal function and serum IGF, APN, leptin levels, reduce bone cement leakage, and have good safety.
