摘要:
目的 观察股骨近端防旋髓内钉（PFNA）内固定治疗股骨粗隆间骨折的临床疗效。方法 选取2016年3月-2018年8月我院治疗的60例股骨粗隆间骨折患者为研究对象，采用随机数字表法分为对照组和观察组，各30例。对照组采用动力髋螺钉（DHS）内固定治疗，观察组采用PFNA治疗，比较两组骨折愈合优良率、临床手术指标、髋关节Harris评分、行走能力评分以及并发症发生情况。结果 观察组骨折愈合优良率为96.67%，高于对照组的83.33%，差异有统计学意义（P＜0.05）；观察组手术时间、术中出血量、负重站立时间、骨折愈合时间小于对照组，差异有统计学意义（P＜0.05）；观察组髋关节Harris评分、行走能力评分高于对照组，差异有统计学意义（P＜0.05）；随访2年，观察组并发症发生率为6.67%，低于对照组的16.67%，差异有统计学意义（P＜0.05）。结论 PFNA内固定治疗股骨粗隆间骨折临床优良率高，术后髋关节功能恢复良好，具有手术时间短、术中出血量少、骨折愈合时间短等优势，且随访远期并发症发生率低。
Abstract:
Objective To observe the clinical effect of proximal femoral nail anti-rotation (PFNA) internal fixation in the treatment of intertrochanteric fracture.Methods A total of 60 patients with femoral intertrochanteric fractures treated in our hospital from March 2016 to August 2018 were selected as the research objects. They were divided into control group and observation group by random number table method, with 30 cases in each group. The control group was treated with dynamic hip screw (DHS) internal fixation, and the observation group was treated with PFNA. The excellent and good rate of fracture healing, clinical operation indexes, Harris hip score, walking ability score and complications of the two groups were compared.Results The excellent and good rate of fracture healing in the observation group was 96.67%, which was higher than 83.33% in the control group, and the difference was statistically significant (P<0.05). The operation time, intraoperative blood loss, weight-bearing standing time and fracture healing time in the observation group were less than those in the control group, and the difference was statistically significant (P<0.05). The Harris score and walking ability score of the hip joint in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). After 2 years of follow-up, the incidence of complications in the observation group was 6.67%, which was lower than 16.67% in the control group, and the difference was statistically significant (P<0.05).Conclusion PFNA internal fixation has high clinical excellent and good rate in the treatment of femoral intertrochanteric fractures, has the advantages of short operation time, less intraoperative blood loss, short fracture healing time, and low incidence of long-term complications after follow-up.
