摘要:
目的 探究连续血液净化(CBP)联合血必净对急性呼吸窘迫综合征(ARDS)患者氧化应激及急性生理与慢性健康评分(APACHEⅡ)的影响。方法 选取2019年1月-2021年1月我院收治的92例ARDS患者,随机分为对照组与观察组,各46例。对照组采用CBP治疗,观察组在CBP基础上联合血必净治疗,比较两组治疗前后血气分析指标[动脉氧分压(PaO2)、氧合指数(P/F)]、炎症反应指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、氧化应激指标[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH)、丙二醛(MDA)]及APACHEⅡ评分。结果 观察组PaO2、P/F指标高于对照组,差异有统计学意义(P<0.05);观察组血清TNF-α、IL-6水平低于对照组,差异有统计学意义(P<0.05);观察组血清SOD、GSH含量高于对照组,MDA含量低于对照组,差异有统计学意义(P<0.05);观察组APACHEⅡ评分低于对照组,差异有统计学意义(P<0.05)。结论 CBP联合血必净可改善ARDS患者的血氧状态,缓解其炎症反应与氧化应激损伤,促进机体生理功能的恢复。
Abstract:
Objective To explore the effect of continuous blood purification (CBP) combined with Xuebijing on oxidative stress and acute physiology and chronic health score (APACHEⅡ) in patients with acute respiratory distress syndrome (ARDS).Methods A total of 92 patients with ARDS admitted to our hospital from January 2019 to January 2021 were randomly divided into control group and observation group, with 46 cases in each group. The control group was treated with CBP, and the observation group was treated with Xuebijing on the basis of CBP. The blood gas analysis indexes [arterial partial pressure of oxygen (PaO2), oxygenation index (P/F)], inflammatory response indexes [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)], oxidative stress indexes [superoxide dismutase (SOD), glutathione peroxidase (GSH), malondialdehyde (MDA)] and APACHE Ⅱ score were compared between the two groups before and after treatment.Results PaO2 and P/F in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). The levels of serum TNF-α and IL-6 in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). The serum SOD and GSH contents in the observation group were higher than those in the control group, and MDA content was lower than that in the control group, with significant difference (P<0.05). The APACHE Ⅱ score of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05).Conclusion CBP combined with Xuebijing can improve the blood oxygen status of ARDS patients, alleviate the inflammatory response and oxidative stress injury, and promote the recovery of physiological function.
