摘要:
目的 研究同期与分期肺结节手术对双侧肺结节患者术后肺功能及血气指标的影响。方法 选取2019年2月-2021年2月佳木斯市肿瘤医院胸外科收治的58例双侧肺结节患者作为研究对象，依据手术方案的不同分为同期组（同期肺结节手术治疗，29例）与分期组（分期肺结节手术治疗，29例），比较两组手术指标、术后并发症、肺功能[用力肺活量占预计值百分比（FVC%）、每分钟最大通气量（MVV）]、血气指标[血氧分压（PaO2）、二氧化碳分压（PaCO2）]以及预后情况。结果 同期组总手术时间、胸管携带时间以及总住院时间均短于分期组（P＜0.05）。同期组与分期组术后1个月术后并发症发生率比较，差异无统计学意义（P＞0.05）。同期组术后1周FVC%、MVV、PaO2低于分期组，PaCO2高于分期组（P＜0.05）。结论 同期手术可缩短双侧肺结节患者在院治疗时间，且不会引起术后并发症风险的升高，短期预后良好，但术后肺功能损伤大，适用范围有限。
Abstract:
Objective To study the effects of simultaneous and staged pulmonary nodule surgery on postoperative pulmonary function and blood gas indexes of patients with bilateral pulmonary nodules.Methods A total of 58 patients with bilateral pulmonary nodules admitted to the Department of Thoracic Surgery, Jiamusi Tumor Hospital from February 2019 to February 2021 were selected as the research objects. According to the different surgical schemes, they were divided into the same period group (simultaneous pulmonary nodule surgery, 29 cases) and the staging group (staged pulmonary nodule surgery, 29 cases). The surgical indexes, postoperative complications, pulmonary function [forced vital capacity (FVC%), maximum ventilation per minute (MVV)], blood gas indexes [partial pressure of blood oxygen (PaO2), carbon dioxide partial pressure (PaCO2)] and prognosis were compared between the two groups.Results The total operation time, chest tube carrying time and total hospitalization time in the same period were shorter than those in the staging group (P<0.05). There was no significant difference in the incidence of postoperative complications 1 month after operation between the same period group and the staging group (P>0.05). FVC%, MVV and PaO2 at 1 week after operation in the same period group were lower than those in the staging group, and PaCO2 was higher than that in the staging group (P<0.05). Conclusion Simultaneous surgery can shorten the hospitalization time of patients with bilateral pulmonary nodules, and does not increase the risk of postoperative complications. The short-term prognosis is good, but the postoperative pulmonary function damage is large and the scope of application is limited.
