摘要:
目的 探讨RhCcEe抗原配合性输血在多次输血患者中的应用价值。方法 选取2017年6月-2020年6月在西南医科大学附属医院接受多次输血治疗的患者155例作为研究对象,按照随机数字表法分成试验组77例和对照组78例。试验组进行ABO、RhCcEe抗原配合性输血,对照组仅进行ABO、RhD相合性输血,比较两组输血后不规则抗体产生以及不良反应发生情况。结果 Rh抗原表型分布为CCDee(47.74%)、CcDEe(31.61%)、CcDee(9.03%)、ccDEE(6.45%)、ccDEe(3.87%)、CCDEe(1.30%)。试验组不规则抗体发生率为1.30%,低于对照组的10.26%,差异有统计学意义(P<0.05);试验组未发生输血不良反应,而对照组有6例输血不良反应,以迟发性的血清学反应为主,组间比较,差异有统计学意义(P<0.05)。结论 对需要反复多次输血的患者进行RhCcEe抗原配合性输血可有效预防和减少患者Rh血型系统不规则抗体的产生,降低输血不良反应率,确保血液输注的有效性以及安全性。
Abstract:
Objective To explore the application of RhCcEe compatibility management in blood transfusion of patients with multiple transfusions.Methods A total of 155 patients who received multiple transfusions in the Department of Hematology, the Affiliated Hospital of Southwest Medical University from June 2017 to June 2020 were selected as the study subjects. According to the random number table method, they were divided into experimental group (77 cases) and control group (78 cases). The experimental group received ABO and RhCcEe compatibility management in blood transfusion, while the control group only received ABO and RhD consistent blood transfusion. The irregular antibody production and adverse reactions after blood transfusion were compared between the two groups.Results The RhCcEe phenotype distribution was CCDee (47.74%), CcDEe(31.61%), CcDee(9.03%), ccDEE(6.45%), ccDEe(3.87%) and CCDEe(1.30%). The incidence of irregular antibodies in the experimental group was 1.30%, which was lower than 10.26% in the control group, and the difference was statistically significant (P<0.05). The experimental group had no adverse transfusion reactions, while the control group had 6 cases of adverse transfusion reactions, mainly delayed serological reactions, and the difference was statistically significant (P<0.05).Conclusion RhCcEe compatibility management is helpful to reduce the rate of irregular antibody production and the incidence of adverse reactions after transfusion in patients with multiple transfusion, and ensure the effectiveness and safety of blood transfusion.
