摘要:
目的 观察康复护理预防颈椎骨折伴脊髓损伤患者术后下肢深静脉血栓形成的效果。方法 选取2020年3月-2021年5月在我院诊治的64例颈椎骨折伴脊髓损伤患者为研究对象，采用随机数字表法分为对照组和观察组，各32例。对照组采用常规护理，观察组采用康复护理，比较两组患者下肢深静脉血栓发生率、肺栓塞发生率、不同时间段抗凝血酶活性、凝血和纤溶功能指标、护理满意度。结果 观察组下肢深静脉血栓发生率为3.13%、肺栓塞发生率为0，均低于对照组的15.63%、9.38%，差异有统计学意义（P＜0.05）；观察组术后3、5、10 d抗凝血酶活性均大于对照组，差异有统计学意义（P＜0.05）；观察组血小板计数（BPC）低于对照组，凝血酶时间（TT）、凝血酶原时间（PT）、纤维蛋白降解产物（FDP）和D-二聚体（D-D）均高于对照组，差异有统计学意义（P＜0.05）；观察组护理满意度为96.88%，高于对照组的84.38%，差异有统计学意义（P＜0.05）。结论 康复护理可有效预防颈椎骨折伴脊髓损伤患者术后下肢深静脉血栓形成，降低下肢深静脉血栓、肺栓塞的发生率，提高护理满意度，改善凝血功能指标，增强抗凝血酶活性，应用效果确切。
Abstract:
Objective To observe the effect of rehabilitation nursing on prevention of postoperative lower extremity deep venous thrombosis in patients with cervical spine fracture and spinal cord injury.Methods A total of 64 patients with cervical fracture and spinal cord injury who were treated in our hospital from March 2020 to May 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 32 cases in each group. The control group was given routine nursing, and the observation group was given rehabilitation nursing. The incidence of deep venous thrombosis of lower limbs, incidence of pulmonary embolism, antithrombin activity at different time periods, coagulation and fibrinolysis function indexes and nursing satisfaction were compared between the two groups.Results The incidence of lower extremity deep venous thrombosis and pulmonary embolism in the observation group was 3.13% and 0, which were lower than 15.63% and 9.38% in the control group, and the difference was statistically significant (P<0.05). The activities of antithrombin in the observation group at 3, 5, and 10 d after operation were higher than those in the control group, and the difference was statistically significant (P<0.05). The platelet count (BPC) in the observation group was lower than that in the control group, while thrombin time (TT), prothrombin time (PT), fibrin degradation product (FDP) and D-dimer (D-D) in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The nursing satisfaction of the observation group was 96.88%, which was higher than 84.38% of the control group, and the difference was statistically significant (P<0.05).Conclusion Rehabilitation nursing can effectively prevent postoperative lower extremity deep venous thrombosis in patients with cervical fracture and spinal cord injury, reduce the incidence of lower extremity deep venous thrombosis and pulmonary embolism, improve nursing satisfaction, improve coagulation function indexes, and enhance antithrombin activity, which has the exact application effect.
