摘要:
目的 研究微创经皮椎弓根螺钉内固定术对脊柱骨折患者康复速度的影响。方法 选择2019年1月-2020年12月本院收治的120例脊柱骨折患者为研究对象,随机分为微创组和开放组,各60例。开放组实施开放椎弓根螺钉内固定术,微创组实施微创经皮椎弓根螺钉内固定术。比较两组围术期指标、康复指标、超敏C反应蛋白(hs-CRP)、白介素-1(IL-1)水平及并发症发生率。结果 微创组切口长度、骨折愈合时间、住院时间短于开放组,差异有统计学意义(P<0.05);微创组术后引流量低于开放组,差异有统计学意义(P<0.05);术后7 d,两组椎体Cobb角、椎间隙高度、椎体矢状面指数均增加,且微创组高于开放组,差异有统计学意义(P<0.05);两组hs-CRP、IL-1均降低,且微创组低于开放组,差异有统计学意义(P<0.05);微创组并发症发生率为6.67%,低于开放组的21.67%,差异有统计学意义(P<0.05)。结论 脊柱骨折患者应用微创经皮椎弓根螺钉内固定术治疗效果理想,该法对患者机体创伤小,可减轻应激反应,减少出血量,缩短椎体功能康复时间,降低术后并发症。
Abstract:
Objective To study the effect of minimally invasive percutaneous pedicle screw fixation on the rehabilitation speed of patients with spinal fracture.Methods A total of 120 patients with spinal fractures admitted to our hospital from January 2019 to December 2020 were randomly divided into minimally invasive group and open group, with 60 cases in each group. Open pedicle screw fixation was performed in the open group, and minimally invasive percutaneous pedicle screw fixation was performed in the minimally invasive group. Perioperative indicators, rehabilitation indicators, high-sensitivity C-reactive protein (hs-CRP), interleukin-1 (IL-1) levels and complication rates were compared between the two groups.Results The incision length, fracture healing time and hospitalization time in the minimally invasive group were shorter than those in the open group, and the differences were statistically significant (P<0.05). The postoperative drainage volume of the minimally invasive group was lower than that of the open group, and the difference was statistically significant (P<0.05). At 7 days after operation, the Cobb angle, intervertebral space height and vertebral sagittal index of the two groups increased, and those in the minimally invasive group were higher than the open group, the differences were statistically significant (P<0.05). The levels of hs-CRP and IL-1 in the two groups were decreased, and the levels in the minimally invasive group were lower than those in the open group (P<0.05). The incidence of complications in the minimally invasive group was 6.67%, which was lower than 21.67% in the open group, and the difference was statistically significant (P<0.05).Conclusion Minimally invasive percutaneous pedicle screw fixation is effective in the treatment of patients with spinal fractures. This method has less trauma to the patient’s body, can reduce stress response, reduce blood loss, shorten the recovery time of vertebral function, and reduce postoperative complications.
