摘要:
目的 研究小剂量胺碘酮与美托洛尔治疗老年性房颤的效果及对左心房和心功能的影响。方法 选取2018年2月-2021年2月我院收治的122例老年性房颤患者作为研究对象，根据随机数字表法分为对照组和观察组，各61例。对照组采用胺碘酮治疗，观察组采用小剂量胺碘酮联合美托洛尔治疗，比较两组临床疗效、左心室重构指标（左心房内径、左心房容积指数、左心室射血分数）、心功能指标[心搏量（SV）、心输出量（CO）、左心室舒张早期/晚期速度峰值比值（VA/VE）]、不良反应、房颤复发情况。结果 观察组治疗总有效率为95.08%，高于对照组的83.61%（P＜0.05）；观察组左心房内径、左心房容积指数低于对照组，左心室射血分数高于对照组（P＜0.05）；观察组SV、CO高于对照组，VA/VE值低于对照组（P＜0.05）；观察组CK、CK-MB、BNP水平低于对照组（P＜0.05）；两组不良反应发生率比较，差异无统计学意义（P＞0.05）；观察组房颤复发率低于对照组（P＜0.05）。结论 小剂量胺碘酮与美托洛尔治疗老年性房颤疗效确切，可抑制左心房重构，改善心功能，减少心肌损伤及房颤复发情况，且不增加不良反应发生率。
Abstract:
Objective To study the effect of low-dose amiodarone and metoprolol in the treatment of senile atrial fibrillation and its effect on left atrium and cardiac function.Methods A total of 122 elderly patients with atrial fibrillation admitted to our hospital from February 2018 to February 2021 were selected as the research objects. According to the random number table method, they were divided into control group and observation group, with 61 cases in each group. The control group was treated with amiodarone, while the observation group was treated with low-dose amiodarone combined with metoprolol. The clinical efficacy, left ventricular remodeling indexes (left atrial diameter, left atrial volume index, left ventricular ejection fraction), cardiac function indexes [stroke volume (SV), cardiac output (CO), left ventricular early diastolic/late diastolic peak velocity ratio (VA/VE)], adverse reactions and recurrence of atrial fibrillation were compared between the two groups.Results The total effective rate of the observation group was 95.08%, which was higher than 83.61% of the control group (P<0.05). The left atrial diameter and left atrial volume index in the observation group were lower than those in the control group, and the left ventricular ejection fraction was higher than that in the control group (P<0.05). SV and CO in the observation group were higher than those in the control group, and VA/VE value was lower than that in the control group (P<0.05). The levels of CK, CK-MB and BNP in the observation group were lower than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). The recurrence rate of atrial fibrillation in the observation group was lower than that in the control group (P<0.05).Conclusion Low-dose amiodarone and metoprolol are effective in treating senile atrial fibrillation, which can inhibit left atrial remodeling, improve heart function, reduce myocardial damage and recurrence of atrial fibrillation, and do not increase the incidence of adverse reactions.
