目的 分析阴道分泌物功能酶测定与常规镜检结合在阴道微生态改变中的应用价值。方法 抽选2019年1月-2021年1月于本院妇产科就诊的自感阴道不适患者66例,对其进行阴道分泌物功能酶[过氧化氢(H2O2)、白细胞酯酶(LE)、β-葡萄糖醛酸苷酶(GUS)、唾液酸酶(Sna)、凝固酶(GADP)]检测与常规镜检测,观察不同年龄段(<20岁、20~39岁、30~39岁、40~49岁、≥50岁)患者的H2O2、LE、GUS、Sna、GADP阳性检出率,不同阴道清洁度患者的H2O2、LE、GUS、Sna、GADP阳性检出率以及GADP+H2O2、GUS+H2O2、GUS+GADP、Sna+H2O2阳性检出率。结果 随着年龄增加,H2O2阳性率升高(P<0.05);≥50岁年龄段LE阳性率低于其他年龄段(P<0.05);其余各年龄段GUS、Sna、GADP阳性率比较,差异无统计学意义(P>0.05)。Ⅲ~Ⅳ度患者H2O2、Sna阳性率高于Ⅰ~Ⅱ度(P<0.05);不同阴道清洁度LE、GUS、GADP阳性率比较,差异无统计学意义(P>0.05)。Ⅲ~Ⅳ度Sna+H2O2、GUS+H2O2、GADP+H2O2阳性率高于Ⅰ~Ⅱ度(P<0.05);不同阴道清洁度GUS+GADP阳性率比较,差异无统计学意义(P>0.05)。结论 阴道分泌物功能酶测定与常规镜检结合有助于判断阴道微生态改变,阴道分泌物功能酶之间的联合判断可提前预测阴道微生态变化,对防治阴道疾病具有重要意义。
Abstract:
Objective To analyze the application value of vaginal secretion functional enzyme determination combined with routine microscopy in vaginal microecological changes.Methods Sixty-six patients with self-perceived vaginal discomfort who were treated in the obstetrics and gynecology department of our hospital from January 2019 to January 2021 were selected for the detection of vaginal secretion functional enzymes [hydrogen peroxide (H2O2), leukocyte esterase (LE), β-glucuronidase (GUS), sialidase (Sna), and coagulase (GADP)] and routine microscopic examination. The positive rates of H2O2, LE, GUS, Sna, and GADP in patients of different ages (<20 years, 20-39 years, 30-39 years, 40-49 years, ≥50 years) were observed. The positive rates of H2O2, LE, GUS, Sna, GADP and GADP+ H2O2, GUS+H2O2, GUS+GADP, Sna+H2O2 in patients with different vaginal cleanliness were detected.Results The positive rate of H2O2 increased with age (P<0.05); the positive rate of LE in≥50 years old group was lower than that in other age groups (P<0.05); there was no significant difference in the positive rates of GUS, Sna and GADP among other age groups (P>0.05). The positive rate of H2O2 and Sna in Ⅲ-Ⅳ degree patients was higher than that in Ⅰ-Ⅱ degree patients(P<0.05); there was no significant difference in the positive rates of LE, GUS and GADP among different vaginal cleanliness (P>0.05). The positive rates of Sna+H2O2, GUS +H2O2 and GADP+H2O2 in Ⅲ-Ⅳ degrees were higher than those in Ⅰ-Ⅱ degrees (P<0.05); there was no significant difference in the positive rate of GUS+GADP in different vaginal cleanliness (P>0.05).Conclusion The combination of vaginal secretion functional enzyme determination and routine microscopy is helpful to judge the changes of vaginal microecology. The combined judgment of vaginal secretion functional enzymes can predict the changes of vaginal microecology in advance, which is of great significance for the prevention and treatment of vaginal diseases.
