摘要:
目的 探究神经节苷脂治疗新生儿缺氧缺血性脑病(HIE)的临床效果及其对肿瘤坏死因子α(TNF-α)和白细胞介素6(IL-6)的影响。方法 选取2020年1月-2021年12月佳木斯市中心医院收治的50例HIE患儿,经随机数字表法分为对照组(25例)和观察组(25例)。对照组行常规治疗,观察组在其基础上加用神经节苷脂治疗,比较两组临床疗效、血气指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、血氧饱和度(SaO2)]、血清炎性因子[TNF-α、IL-6、超敏C反应蛋白(hs-CRP)]、新生儿神经行为测定(NBNA)评分。结果 观察组治疗有效率较对照组高(P<0.05);两组治疗后PaO2、SaO2升高,PaCO2下降,且观察组PaO2、SaO2高于对照组,PaCO2低于对照组(P<0.05);两组治疗后TNF-α、IL-6、hs-CRP均有下降,且观察组低于对照组(P<0.05);两组治疗后NBNA评分均有升高,且观察组高于对照组(P<0.05)。结论 神经节苷脂治疗HIE疗效确切,可改善患儿血氧状态,减轻炎性反应,促使神经修复。
Abstract:
Objective To investigate the clinical effect of ganglioside in the treatment of neonatal hypoxic ischemic encephalopathy (HIE) and its effect on tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6).Methods Fifty children with HIE admitted to Jiamusi Central Hospital from January 2020 to December 2021 were selected and divided into control group (25 cases) and observation group (25 cases) by random number table method. The control group was treated with routine treatment, and the observation group was treated with ganglioside on the basis of the control group. The clinical efficacy, blood gas indexes [arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), saturation of blood oxygen (SaO2)], serum inflammatory factors [TNF-α, IL-6, hypersensitive C-reactive protein (hs-CRP)] and neonatal behavioral neurological assessment (NBNA) score were compared between the two groups.Results The effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, PaO2 and SaO2 increased and PaCO2 decreased in the two groups, and PaO2 and SaO2 in the observation group were higher than those in the control group, and PaCO2 was lower than that in the control group (P<0.05)). After treatment, TNF-α, IL-6 and hs-CRP in the two groups decreased, and those in the observation group were lower than those in the control group (P<0.05). The NBNA scores of the two groups increased after treatment, and those in the observation group were higher than those in the control group (P<0.05).Conclusion Ganglioside is effective in the treatment of HIE, which can improve the blood oxygen state, reduce the inflammatory reaction and promote nerve repair.
