摘要:
目的 评价糖链抗原125（CA125）、人附睾蛋白4（HE4）并联检测在复发性卵巢癌诊断中的价值。方法 计算机检索Pubmed、CBMdisc、CNKI、维普等数据库，收集关于复发性卵巢癌诊断中并联检测CA125、HE4的文献。由2名评价人员独立根据QUADAS 2工具对文献质量进行评价，使用Meta-DiSc 14.0、Stata16.0软件进行Meta分析。结果 按照统一的纳入标准和排除标准共获得10篇文献。结果显示在复发性卵巢癌诊断中，对比单独检测CA125，卵巢癌已正规治疗（手术+标准化疗）(组1)单独检测HE4的合并SE低0.01，合并SP高0.08，合并+LR高0.65，合并-LR低0.06，合并AUC高0.0902，合并Q指数高0.0744，合并诊断比值比高2.79；上皮性卵巢癌已正规治疗（手术+标准化疗）（组2）联合检测CA125+HE4合并SE最高，单独检测HE4合并SP最高，联合检测CA125+HE4合并+LR、-LR较高，联合检测CA125+HE4合并AUC、Q指数较高。结论 复发性卵巢癌的临床辅助诊断中，联合CA125+HE4检测或单独检测HE4均较单独检测CA125判别能力强，提示HE4在诊断复发性卵巢癌上有一定的临床价值。
Abstract:
Objective To evaluate the value of combined detection of carbohydrate antigen 125 (CA125) and human epididymis protein 4 (HE4) in the diagnosis of recurrent ovarian cancer.Methods Pubmed, CBMdisc, CNKI, VIP and other databases were searched by computer to collect the literature on parallel detection of CA125 and HE4 in the diagnosis of recurrent ovarian cancer. Two reviewers independently evaluated the quality of the literature according to the QUADAS 2 tool, and Meta-analysis was performed using Meta-DiSc 14.0 and Stata16.0 software.Results Ten literatures were obtained according to uniform inclusion and exclusion criteria. The results showed that in the diagnosis of recurrent ovarian cancer, compared with CA125 alone, combined SE of HE4 alone in ovarian cancer had been regularly treated (surgery+standard chemotherapy) (group 1) was 0.01 lower, combined SP was 0.08 higher, combined+LR was 0.65 higher, combined -LR was 0.06 lower, combined AUC was 0.0902 higher, combined Q index was 0.0744 higher, and combined diagnostic odds ratio was 2.79 higher; epithelial ovarian cancer had been regularly treated (surgery+standard chemotherapy) (group 2): CA125+HE4 combined with SE was the highest, HE4 combined with SP was the highest, CA125+HE4 combined with LR and - LR were higher, and CA125+HE4 combined with AUC and Q index were higher.Conclusion In the clinical auxiliary diagnosis of recurrent ovarian cancer, the results show that the combined detection of CA125+HE4 or the detection of HE4 alone is more discriminative than the detection of CA125 alone, suggesting that HE4 has certain clinical value in the diagnosis of recurrent ovarian cancer.
