摘要:
目的 分析集束化护理预防重症医学科（ICU）患者呼吸机相关性肺炎发生的效果。方法 选择2017年6月~2019年6月在我院ICU治疗的患者130例，采用随机数字表法分为对照组和观察组，各65例。对照组采用常规护理，观察组在对照组基础上实施集束化护理，比较两组呼吸机相关性肺炎发生情况、气管切开率、撤机成功率、机械通气时间、ICU入住时间、住院时间。结果 观察组呼吸机相关性肺炎发生率为13.84%、气管切开率为23.07%，分别低于对照组的29.23%、58.46%，差异有统计学意义（P＜0.05）；观察组撤机成功率为83.07%，高于对照组的26.15%，差异有统计学意义（P＜0.05）；观察组机械通气时间为（8.18±2.69）d、ICU入住时间为（14.55±5.11）d、住院时间为（12.11±4.78）d，分别短于对照组的（13.87±3.06）d、（18.41±4.89）d、（21.21±5.03）d，差异有统计学意义（P＜0.05）。结论 集束化护理在预防ICU患者呼吸机相关性肺炎发生方面具有良好的效果，可有效降低呼吸机相关性肺炎发生率，缩短ICU入住时间，进一步促进患者的康复。
Abstract:
Objective To analyze the effect of cluster nursing on prevention of ventilator-associated pneumonia in patients in ICU.Methods From June 2017 to June 2019, 130 patients were treated in the ICU in our hospital, and they were divided into a control group and an observation group with a random number table method, 65 cases each. The control group received routine nursing, and the observation group implemented cluster nursing on the basis of the control group to compare the incidence of ventilator-associated pneumonia, tracheotomy rate, success rate of evacuation, mechanical ventilation time, ICU stay time, and hospital stay.Results The incidence rate of ventilator-associated pneumonia in the observation group was 13.84%, and the tracheotomy rate was 23.07%, which were lower than those in the control group, 29.23% and 58.46%, respectively,the difference was statistically significant(P＜0.05);The success rate of withdrawal in the observation group was 83.07%, higher than that in the control group of 26.15%, the difference was statistically significant (P＜0.05); the mechanical ventilation time of the observation group was (8.18±2.69) d, and the ICU stay time was (14.55±5.11 )d. The hospital stay is (12.11±4.78) d, which is shorter than that of the control group (13.87±3.06) d,(18.41±4.89) d, (21.21±5.03) d, the difference was statistically significant (P＜0.05).Conclusion Cluster nursing has a good effect in preventing the occurrence of ventilator-associated pneumonia in ICU patients, which can effectively reduce the incidence of ventilator-associated pneumonia, shorten the stay time of ICU, and further promote the rehabilitation of patients.
