摘要:
目的 研究依托咪酯靶控输注复合瑞芬太尼诱导对中老年高血压患者血流动力学的影响及临床麻醉效果。方法 选取2018年9月~2019年9月我院手术治疗的98例中老年患者为研究对象,根据是否合并高血压分为观察组(合并高血压)50例和对照组(未合并高血压)48例,均采用依托咪酯靶控输注复合瑞芬太尼诱导,观察比较两组不同时间段[诱导前(T0)、诱导结束(T1)、插管时(T2)、结束时(T3)、结束1 min(T4)、结束5 min(T5)]平均动脉压(MAP)、心率(HR)、术后睁眼时间、定向力恢复时间、拔管时间、麻醉效果以及术后不良反应发生情况。结果 观察组T0、T1、T2时HR与对照组比较,T1、T4、T5时MAP与对照组比较,差异无统计学意义(P>0.05),观察组T3、T4时HR高于对照组,T0、T2、T3时MAP高于对照组(P<0.05);观察组术后睁眼、定向力恢复、拔管时间与对照组比较,差异均无统计学意义(P>0.05);两组麻醉优良率及术后不良反应发生率比较,差异无统计学意义(P>0.05)。结论 依托咪酯靶控输注复合瑞芬太尼诱导对中老年高血压患者麻醉效果良好,血流动力学相对稳定,术后患者苏醒快,麻醉效果显著,且术后临床不良反应少。
Abstract:
Objective To study the effect of etomidate target controlled infusion combined with remifentanil induction on hemodynamics and clinical anesthesia effects in middle-aged and elderly hypertensive patients.Methods From September 2018 to September 2019, 98 middle-aged and elderly patients who were surgically treated in our hospital were selected as the research objects. According to whether they had hypertension or not, they were divided into observation group (with hypertension), 50 cases and control group (without hypertension) 48 cases were induced by etomidate target controlled infusion combined with remifentanil, and the two groups were observed and compared at different time periods [before induction (T0), end of induction (T1), during endotracheal intubation (T2), at the end (T3), 1 min (T4), 5 min (T5)] Mean arterial pressure (MAP), heart rate (HR), postoperative eye opening time, orientation recovery time, extubation time, anesthesia the effect and the occurrence of adverse reactions after surgery.Results The HR of the observation group was compared with the control group at T0, T1, and T2, and the MAP at T1, T4, T5 was compared with the control group,there was no significant difference (P>0.05). The observation group’s HR at T3 and T4 was higher than that of the control group, and the MAP at T0, T2, and T3 was higher than that of the control group (P<0.05); the observation group’s postoperative eye opening, orientation recovery, and extubation time were compared compared with the control group, the difference was not statistically significant (P>0.05); the excellent and good rate of anesthesia and adverse reactions of two group were not statistically significant (P>0.05).Conclusion Etomidate target controlled infusion combined with remifentanil induction had a good anesthesia effect for middle-aged and elderly hypertensive patients, with relatively stable hemodynamics, quick recovery of postoperative patients, significant anesthesia effects, and few postoperative clinical adverse reactions.
