摘要:
目的 比较微型钢板与克氏针在治疗掌骨骨折安全性与有效性。方法 采用循证医学的方法检索CBM、CNKI、VIP、Wanfang、PubMed、Cochrane Library等数据库,文献检索时间区间为建库至2014年3月,并手工检索中华骨科杂志、中华创伤骨科杂志、中华创伤杂志,按照纳入排除标准筛选文献,对纳入文献按照Cochrane手册评价方法进行质量评价,对符合标准的文献进行数据提取,利用RevMan5.2进行数据合并分析。结果 最终纳入14个RCT,共1160例患者。两种手术优良率[OR=4.3791,95%CI(3.1428,6.1016),P<0.00001]、骨折愈合时间[SMD=-1.75,95%CI(-2.85,-0.66),P=0.002]、并发症中肌腱粘连[OR=0.07,95%CI(0.02,0.25),P=0.0001]、感染[OR=0.23,95%CI(0.12,0.42),P<0.0001]、关节僵硬[OR=0.09,95%CI(0.03,0.28),P<0.0001]比较,差异有统计学意义(P<0.05);两组手术时间[SMD=0.63,95%CI(0.01,1.26),P=0.05]、术后住院时间[MD=0.02,95%CI(-0.27,0.32),P=0.88]、延期愈合[OR=0.49,95%CI(0.09,2.74),P=0.42]、畸形愈合[OR=0.33,95%CI(0.03,3.19),P=0.34]比较,差异无统计学意义(P>0.05)。结论 与克氏针相比,微型钢板更为安全有效,主要表现在微型钢板具有更高的手术优良率,骨折愈合时间更短,术后肌肉粘连、关节僵直、感染方面的并发症发生率低,而在手术时间、住院时间、延期愈合、畸形愈合方面无差异。
Abstract:
Objective To systematically review the effectiveness and safety of microplate and Kirschner wire for treating of metacarpal bones fractures. Methods Use evidence-based medicine to search databases such as CBM, CNKI, VIP, Wanfang, PubMed, Cochrane Library, and the literature search time interval is from the establishment of the database to March 2014, and manually search the Chinese Journal of Orthopaedics, Chinese Journal of Traumatology, and Chinese Journal of Trauma,screen the literature according to the inclusion and exclusion criteria, evaluate the quality of the included literature according to the Cochrane Handbook evaluation method, extract data from the literature that meets the criteria, and use RevMan5.2 for data merging analysis.Results Finally, 14 RCTs were included, with a total of 1160 patients. The excellent and good rate of the two operations [OR=4.3791, 95%CI(3.1428, 6.1016),P<0.00001], fracture healing time [SMD=-1.75, 95%CI(-2.85, -0.66),P=0.002], concurrent tendon adhesion [OR=0.07, 95%CI(0.02, 0.25),P=0.0001], infection [OR=0.23, 95%CI(0.12, 0.42),P<0.0001], joint stiffness [OR=0.09, 95%CI(0.03, 0.28),P<0.0001], the difference was statistically significant (P<0.05); the operation time of the two groups [SMD=0.63, 95%CI(0.01, 1.26),P=0.05], surgery post hospital stay [MD=0.02, 95%CI(-0.27, 0.32),P=0.88], delayed healing [OR=0.49, 95%CI(0.09, 2.74),P=0.42], malunion [OR=0.33,95%CI (0.03, 3.19), P=0.34], the difference was not statistically significant (P>0.05).Conclusion Compared with Kirschner wires, the microplate is safer and more effective, mainly because the microplate has a higher rate of surgical excellence, a shorter fracture healing time, and a lower incidence of complications in postoperative muscle adhesion, joint stiffness, and infection. There was no difference in operation time, hospitalization time, delayed healing, and deformity healing.
