摘要:
目的 分析2014年到2018年我院尿培养病原菌的分布变迁趋势、耐药性变化趋势以及不同病原菌耐药性的差异。方法 收集2014年1月~2018年12月广东医科大学附属医院尿路感染患者送检尿液标本中分离菌株5662株,分析不同病原菌5年间的耐药性变化趋势。结果 5年间尿培养病原菌构成前6位分别是大肠埃希菌(40.33%)、肺炎克雷伯菌(8.91%)、铜绿假单胞菌(6.39%)、白色念珠菌(6.25%)、粪肠球菌(5.91%)及屎肠球菌(4.48%);5年间尿培养病原菌菌落分布比较,差异无统计学意义(P>0.05);大肠埃希菌、肺炎克雷伯菌耐药率总体呈上升趋势;与大肠埃希菌相比,肺炎克雷伯菌对哌拉西林/他唑巴坦(11.34% vs 2.22%)、呋喃妥因(42.61% vs 5.00%)、替加环素(48.30% vs 4.11%)以及头孢西丁(29.64% vs 13.63%)的耐药率偏高,差异均有统计学意义(P<0.05);革兰阴性菌多重耐药性总体呈上升趋势,其中鲍曼不动杆菌的多重耐药率最高(>38%)。结论 5年间我院尿培养病原菌总体分布基本不变,但常见病原菌总体耐药率呈上升趋势,可能是长期不合理用药导致了耐药比例和耐药亚群的增加,应该引起重视。
Abstract:
Objective To analyze the distribution trend of pathogenic bacteria in urine culture in our hospital from 2014 to 2018, the trend of drug resistance, and the difference in drug resistance of different pathogens.Methods From January 2014 to December 2018, 5,662 strains isolated from urine specimens submitted by patients with urinary tract infection in the Affiliated Hospital of Guangdong Medical University were collected, and the trend of drug resistance of different pathogens during the five-year period was analyzed.Results The top 6 pathogens in urine culture during the five-year period were Escherichia coli (40.33%), Klebsiella pneumoniae (8.91%), Pseudomonas aeruginosa (6.39%), Candida albicans (6.25%),Enterococcus faecalis (5.91%) and Enterococcus faecium (4.48%); the distribution of pathogenic bacteria in urine culture in 5 years, the difference was not statistically significant (P>0.05);The resistance rate of Escherichia coli and Klebsiella pneumoniae showed an overall upward trend. Compared with Escherichia coli, Klebsiella pneumoniae was resistant to piperacillin/tazobactam (11.34% vs 2.22%) and nitrofurantoin ( 42.61% vs 5.00%), tigecycline (48.30% vs 4.11%) and cefoxitin (29.64% vs 13.63%) have high resistance rates,the differences are statistically significant (P<0.05);The multi-drug resistance of Gram-negative bacteria showed an overall upward trend. Among them, Acinetobacter baumannii had the highest multi-drug resistance rate (>38%).Conclusion The overall distribution of pathogenic bacteria in urine culture in our hospital remained basically unchanged during the five years, but the overall drug resistance rate of common pathogens was on the rise. It may be that long-term irrational use of drugs has led to an increase in the proportion of drug resistance and drug resistance subgroups, which should be paid attention to.
